A healthcare professional reported that during an intraocular lens implant procedure when inserting into the eye, the haptic was broken between the plunger, so the intraocular lens was replaced. The lens was replaced with unspecified lens during the same procedure.Additional information has been received from surgeon, stated that the trailing haptic appeared to be slightly tucked prior to insertion. Because of concerns about advancing the relatively long plunger too far into the eye, the surgeon stopped advancing the plunger midway. Upon resuming advancement, the trailing haptic had fully unfolded. Subsequently, the fully unfolded trailing haptic became trapped between the plunger and cartridge and was torn off during insertion.
