  1. Dec 21, 2021Class IIOpen, Classified

Stent Graft Systems built with specific batches of taper tip assemblies have the potential for the taper tip to detach from the delivery system. Detachment of the taper tip during the implant procedure may lead to a secondary intervention to attempt to remove the taper tip, either by endovascular retrieval or surgical conversion. There is also potential that the taper tip is intentionally left behind in the patient, at the physician's discretion, if not easily removable.

IT WAS REPORTED THAT THE PATIENT PRESENTED TO THE HOSPITAL EMERGENTLY TO THE HOSPITAL WITH A RUPTURED ANEURYSM.  IT WAS STATED THAT THERE WAS A TYPE IB ENDOLEAK AND POTENTIAL HOLE IN THE STENT GRAFT. INTERVENTION WAS PERFORMED WHERE THE PROXIMAL PART OF THE ENDURANT WAS REMOVED VIA OPEN AAA REPAIR.  PER THE PHYSICIAN , THE CAUSE OF THE EVENT WAS ANATOMY. AN ENDURANT II BIFURCATED STENT GRAFT WAS IMPLANTED DURING AN ENDOVASCULAR PROCEDURE FOR THE TREATMENT OF AN ABDOMINAL AORTIC ANEURYSM. IT WAS REPORTED THE PATIENT RETURNED FOR A FOLLOW-UP CT APPROXIMATELY 3Y3M POST INDEX PROCEDURE WHERE A TYPE IA ENDOLEAK WAS OBSERVED. THE PHYSICIAN DECIDED TO IMPLANT A CUFF AND ENDOANCHORS IN ORDER TO TREAT THE ENDOLEAK. THE PHYSICIAN PLACED A 25X25X49 ENDURANT CUFF AT THE LEVEL OF THE LOWEST RENAL AND INTO THE EXISTING BIFURCATED GRAFT. THEN BALLOONED THAT CUFF USING A RELIANT BALLOON, AND PLACED 9 ENDOANCHORS AROUND THE PROXIMAL END OF THE CUFF AND EXISTING GRAFT. ANOTHER AORTAGRAM WAS PERFORMED AND THE ENDOLEAK HAD RESOLVED. AS PER THE PHYSICIAN THE CAUSE OF THE ENDOLEAK COULD NOT BE DETERMINED. NO ADDITIONAL CLINICAL SEQUELAE WAS PROVIDED AND THE PATIENT IS FINE.

AN ENDURANT II STENT GRAFT SYSTEM WAS IMPLANTED IN A PATIENT FOR THE ENDOVASCULAR TREATMENT OF AN UNKNOWN SIZED ABDOMINAL AORTIC ANEURYSM. IT WAS REPORTED THAT THE PATIENT RETURNED APPROXIMATELY SIX YEARS POST THE INDEX PROCEDURE COMPLAINING OF HIP PAIN. A CT WAS PERFORMED WHICH SHOWED THE ANEURYSM WAS NOW 80MM AND A TYPE IA ENDOLEAK IS SUSPECTED. IT WAS ALSO REPORTED THAT THE PATIENT WAS APPARENTLY SYMPTOMATIC WITH RIGHT HIP PAIN AT THE TIME OF THE INDEX PROCEDURE. INTERVENTION WAS PERFORMED AND THE ENDOLEAK WAS FIXED WITH THE IMPLANTATION OF A CUFF AND 4 ENDOANCHORS, A REVIEW OF THE ORIGINAL FILMS SHOWED THAT THE ORIGINAL PLACEMENT WAS 1 CM LOWER THAN THE RENALS AS PER THE PHYSICIAN THE CAUSE OF THE EVENT IS ANATOMY AND NOTED THE AORTIC NECK DIAMETER INCREASED IN SIZE TO CREATE A LEAK. NO ADDITIONAL CLINICAL SEQUALAE WERE PROVIDED AND THE PATIENT IS FINE.
