InjurySep 30, 2026Over-Sensing; Inappropriate/Inadequate Shock/Stimulation; Failure to Read Input Signal
It was reported that the patient had an inappropriate shock. The patient was doing Pilates at the time of the shock. The patient has lost a significant amount of weight since the implant back in 2022. An office device check found low intrinsic amplitudes. The sensing vector was then programmed to a new sensing vector. Later, the patient wanted the system explanted due to an improvement in the ejection fraction. The doctor explanted the entire system. There were no additional adverse patient effects. The system remains implanted.

MAUDE report 50949523 ↗InjurySep 25, 2026Signal Artifact/Noise; Over-Sensing; Inappropriate/Inadequate Shock/Stimulation
It was reported that the patient had an inappropriate shock due to the oversensing of noise. The patent was sitting in a vibrating chair at the time of the shock. Technical Services reviewed the device history and found another inappropriate shock due to noise. There were some untreated episodes due to the same noise. Technical Services discussed some testing and programming options. There were no additional adverse patient effects. The system remains implanted.

MAUDE report 50896270 ↗InjurySep 14, 2026High impedance; Use of Device Problem; Malposition of Device; Impedance Problem
It was reported that this subcutaneous implantable cardioverter defibrillator (S-ICD) and electrode exhibited elevated system impedance measurement of 120 Ohms. A 10J shock was performed, and a delivered shock impedance of 124 Ohms was noted. It was noted that the S-ICD was scheduled for a routine battery replacement, and it was not possible to retrieve device data because the S-ICD was at End of Life (EOL). Technical Services (TS) reviewed next steps for the electrode included surgical revision or defibrillation threshold (DFT) testing. If DFTs were unsuccessful, the options were to surgically revise or consider a different implantable system, as it was possible this may be a higher DFT patient. Fluoroscopic imaging may also be used to look for adipose tissue. Additional information received reported that the electrode had been implanted in a suboptimal location, to the left of the sternum. Subsequently, this electrode was surgically abandoned, and a new S-ICD system was implanted. No additional adverse patient effects were reported.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50748334 ↗InjuryJul 20, 2026Signal Artifact/Noise; Over-Sensing; Inappropriate/Inadequate Shock/Stimulation
IT WAS REPORTED THAT THE PATIENT HAD AN INAPPROPRIATE SHOCK DUE TO THE OVERSENSING OF NOISE. A REVIEW OF THE ELECTROGRAMS FOUND SIGNIFICANT RAIL-TO-RAIL NOISE. THERE WERE SOME STORED UNTREATED EPISODES DUE TO THE SAME RAILING NOISE. THE SENSING VECTOR AT THE TIME OF THE SHOCKS WAS SET TO THE PRIMARY SENSING VECTOR. THE NOISE STARTED SUDDENLY THIS PAST APRIL. THERE WERE X-RAYS DONE WHICH HAD SHADOWING WHICH CAN BE SEEN ON THE ELECTRODE AND NEAR THE EDGE OF THE ELECTRODE BODY. THE THERAPY IS NOW PROGRAMMED OFF. TECHNICAL SERVICES ADVISED SOME PROGRAMMING OPTIONS. THE PATIENT SHOULD COME BACK FOR SYSTEM PROGRAMMING. THERE WERE NO ADDITIONAL ADVERSE PATIENT EFFECTS. THE SYSTEM REMAINS IMPLANTED.

MAUDE report 25876946 ↗MalfunctionJul 9, 2026Failure to Convert Rhythm; Under-Sensing
IT WAS REPORTED THAT THE DEVICE HAD DIFFICULTY CONVERTING AN ARRHYTHMIA. THE PATIENT HAD A VENTRICULAR FIBRILLATION EPISODE WHICH REQUIRED MULTIPLE SHOCKS TO CONVERT. THERE WAS SOME UNDERSENSING POST SHOCK WHICH ADDED ABOUT TEN SECONDS TO THE TIME OF THERAPY. TECHNICAL SERVICES DISCUSSED SOME TESTING OPTIONS. THERE WERE NO ADDITIONAL ADVERSE PATIENT EFFECTS. THE SYSTEM REMAINS IMPLANTED.
