IT WAS REPORTED THAT SCREW AND CAP WERE SEPARATED DURING SURGERY. THERE WAS NO PATIENT SYMPTOM REPORTED. THERE WERE NO FURTHER COMPLICATIONS REPORTED REGARDING THE EVENT.
