Data updated Oct 16, 2026 from official FDA records · Editorial policy

Quick summary

This is a Dräger Infinity M540 patient monitor, a bedside unit that tracks vital signs and sounds alarms. Most of the 63 reports describe the monitor giving no audible beep or shutting down, losing its display output, or freezing, along with dropped connections and blood pressure readings that stopped working. Five reports mention a death and five mention an injury. Two recalls, both closed, covered a software problem that lost stored patient settings and data and alarms that might not sound; the first began in March 2018.

It was reported that on the night of Friday, (b)(6), between 3:00 a.m. and 5:00 a.m., a patient died without any event being reported to the ICS monitoring center "[Infinity Central station (ICS)]". The nurses checked the patient¿s vital signs around 3:00 a.m. and then returned to the patient around 5:00 a.m., at which time they confirmed the patient¿s death. According to the available information, no alarm was triggered or displayed on the monitoring console "[Infinity Acute Care System (IACS)]" during that time.

  • MalfunctionMar 26, 2026Electrical /Electronic Property Problem; No Audible Prompt/Feedback; Output Problem

THE CUSTOMER REPORTED THE M540 MONITOR DID NOT EMIT AN AUDIBLE WARNING SIGNAL, EVEN THOUGH IT HAD NOT BEEN MUTED. THE STAFF ONLY BECAME AWARE OF THE ALARM BECAUSE OF THE SIMULTANEOUS VISUAL SIGNAL. THERE WAS NO ADVERSE PATIENT IMPACT, AS THE ALARM CONDITION WAS DETECTED IN TIME.

  • MalfunctionDec 16, 2025Device Sensing Problem; Device Sensing Problem

IT WAS REPORTED THAT THERE WAS AN INTERMITTENT INABILITY TO OBTAIN (NIBP) NON-INVASIVE BLOOD PRESSURE MEASUREMENTS WITH THE M540 PATIENT MONITOR ON A CRITICALLY ILL INFANT. INITIALLY, NIBP MEASUREMENTS WERE SUCCESSFUL DURING TREATMENT IN THE RESUSCITATION UNIT AND ON THE WARD. APPROXIMATELY 4 HOURS AFTER BIRTH (AROUND 4:00 P.M.), ONLY MEAN ARTERIAL PRESSURE (MAD) READINGS WERE POSSIBLE. AFTER AN ADDITIONAL 2 HOURS (AROUND 6:00 P.M.), NO MEASUREMENTS COULD BE OBTAINED. NORMAL NIBP MEASUREMENT RESUMED APPROXIMATELY 7 HOURS LATER (AROUND 1:00 A.M.) IN THIS CASE THERE WAS NO ADVERSE PATIENT IMPACT. HOWEVER, THE INABILITY TO OBTAIN THE DESIRED PARAMETER READING USING THE INFINITY ACUTE CARE SYSTEM ON A CRITICAL PATIENT HAS THE POTENTIAL TO CAUSE OR CONTRIBUTE TO A DEATH OR SERIOUS INJURY IF THIS MALFUNCTION WERE TO RECUR.

  • MalfunctionDec 15, 2025No Device Output; No Device Output

IT WAS REPORTED THAT THE NIBP (NON-INVASIVE BLOOD PRESSURE) MEASUREMENT FEATURE ON THE IACS (INFINITY ACUTE CARE SYSTEM) M540 PATIENT MONITOR REMAINED NON-FUNCTIONAL FOR NEONATES WEIGHING LESS THAN 1000 GRAMS. ON (B)(6) 2025, 12:30 A NEONATE WEIGHING 780 GRAMS REQUIRED CONTINUOUS MONITORING. DUE TO THE INABILITY TO OBTAIN NIBP READINGS WITH THE IACS, A PHILIPS MONITOR WAS USED TO ENSURE CONTINUOUS MEASUREMENT. ON (B)(6), ONCE THE NEONATE¿S CONDITION STABILIZED AND VITAL SIGNS IMPROVED, NIBP MEASUREMENTS COULD AGAIN BE PERFORMED USING THE IACS. NO PATIENT HARM OCCURRED DURING THIS PERIOD; HOWEVER, THE INABILITY TO MEASURE NIBP IN EXTREMELY LOW-WEIGHT NEONATES PRESENTS A POTENTIAL RISK FOR DELAYED CLINICAL INTERVENTION.

IT WAS REPORTED ON (B)(6) 2025, AT APPROXIMATELY 04:18 A.M., A VENTRICULAR TACHYCARDIA (VT) ALARM LASTING APPROXIMATELY NINE MINUTES WAS NOT DISPLAYED AT THE CENTRAL MONITORING STATION. THE PATIENT PARAMETERS, INCLUDING ELECTROCARDIOGRAM (ECG) VALUES, WERE TRANSMITTED AND DISPLAYED CORRECTLY AT THE CENTRAL STATION; HOWEVER, NO ALARM NOTIFICATION WAS GENERATED OR DISPLAYED DURING THE REPORTED TIMEFRAME. THERE WAS NO ADVERSE PATIENT IMPACT.

Patient problems: No Clinical Signs, Symptoms or Conditions; Tachycardia

  • MalfunctionNov 3, 2025Low Readings; Low Readings

IT WAS REPORTED THE NON-INVASIVE BLOOD PRESSURE (NIBP) CUFF WAS USED ON A PATIENT TO OBTAIN A NIBP READING. THE READING WAS LOW BUT WITHIN THE MINIMUM ACCEPTABLE RANGE, SO THE NURSE DECIDED NOT TO GIVE THE PATIENT FENTANYL BOLUSES FOR PAIN MANAGEMENT. THE FOLLOWING DAY, THE SAME CUFF WAS USED TO OBTAIN ANOTHER NIBP READING BUT THE READING WAS VERY LOW, WHICH PROMPTED THE USER TO INSPECT THE CUFF. DURING THEIR INSPECTION IT WAS FOUND THE CUFF HAD SMALL TEARS ALONG THE VELCRO. THE USER EXPRESSED CONCERNS THAT THESE TEARS COULD HAVE IMPACTED THE ACCURACY OF THE NIBP READING. NO ADVERSE PATIENT IMPACT WAS REPORTED.

AS SHOWN IN THE PROVIDED VIDEO, THE ECG WAVEFORMS WERE NOT DISPLAYED, BUT THE NUMERIC VALUES OF THE ECG PARAMETERS WERE STILL AVAILABLE. THE CABLES WERE EXCHANGED AS AN ATTEMPT TO RESOLVE THE ISSUE, BUT THE ISSUE PERSISTED. THEY THEN DECIDED TO CHANGE THE AUTODETECTION TO 3 LEAD AND THE ECG WAVEFORMS RETURNED. A 10-MINUTE DELAY IN PATIENT CARE WAS REPORTED BUT THERE WAS NO ADVERSE PATIENT IMPACT AS A RESULT OF THIS DELAY.
