Trima Accel® 82520Recalls & reported problems
Terumo Bct, Inc. · Disposable Tubing Sets, Auto RBC, Plasma Set · Class 2
Quick summary
This Terumo BCT Trima Accel disposable tubing set is used to collect blood cell components, separating red cells and plasma from a donor. Most of the 53 reports describe problems with collected product quality — red blood cells breaking down (hemolysis) and leftover white blood cells showing up in the product — rather than device breakdowns; many reports simply lack detail about what went wrong. Two injuries and no deaths were reported. No recalls name this device.

THE CUSTOMER REPORTED THAT DURING A DOUBLE RED BLOOD CELL PROCEDURE (AUTOMATED 2RBC) HEMOLYSIS WAS OBSERVED AFTER MIDWAY LINE SWITCH (AFTER BAG 1 WAS FILLED, AFTER LINE 2 COLLECTED SOME BLOOD). IT WAS NOTICED IN THE TUBING LINE/RESERVOIR. NO CLOTS WERE OBSERVED IN THE CHANNEL OR CHANNEL LINES. ANOTHER TECHNICIAN RESPONDED TO AN ALARM, BUT PER THE CUSTOMER IT WAS UNCLEAR WHICH ALARM NUMBER PROMPTED THEM TO CHECK; IT WAS SHORTLY AFTER THE LINES WERE SWITCHED TO COLLECT THE SECOND BAG. THEY NOTICED THE RESERVOIR CONTAINED SEPARATED PLASMA AND RED CELLS TOWARDS THE BOTTOM OF THE RESERVOIR. UPON RETURN TO THE DONOR, IT WAS NOTICED THERE WAS SETTLED RED CELLS AT THE BOTTOM OF THE RETURN LINE AND A "STRIPEY" PATTERN OF FLUIDS RETURNING TO THE DONOR. SEEING "CLUMPED" RED CELLS. THEY ENDED THE DONATION IMMEDIATELY AS IT SEEMED MORE LIKE HEMOLYSIS THAN RED CELL SPILLOVER. NO HEMOLYSIS TESTING WAS PERFORMED. THE PLASMA COLOR WAS YELLOW IN THE RESERVOIR, LIGHT PINK/ORANGE IN THE LINE BUT IT MOSTLY LOOKED SEPARATED. NO MEDICAL INTERVENTION WAS REQUIRED FOR THIS EVENT. FULL PATIENT ID: (B)(6) THE COLLECTION SET IS NOT AVAILABLE FOR RETURN BECAUSE IT WAS DISCARDED BY THE CUSTOMER.
