PneumoLiner WA90500USRecalls & reported problems
Advanced Surgical Concepts Limited · The PneumoLiner device is intended for use as a multiple instrument port and tissue containment system during minimally invasive gynecologic laparoscopic surgery to enable the isolation and containment of tissue, considered benign, resected during single-port or multi-site laparoscopic surgery during power morcellation and removal. When used in women with fibroids, the PneumoLiner is for women who are pre-menopausal and under age 50. The PneumoLiner is compatible with bipolar or electromechanical laparoscopic power morcellators that are between 15mm and 18mm in shaft outer diameter and 135mm and 180mm in shaft working length and which have an external component that allows for the proper orientation of the laparoscope to perform a contained morcellation. The ASC PneumoLiner Device has two main components. It consists of: 1. The PneumoLiner, a tissue pouch intended to provide a space within the abdomen for the safe morcellation of tissue; and 2. A laparoscopic multi-instrument port. 1. Multi-instrument Port The laparoscopic multi-instrument port is comprised of the following three sub-components: The Retractor Introducer which delivers the Distal Ring of the Retractor through a premade incision, into the abdominal cavity. The Retractor which retracts an abdominal incision to allow the passage of laparoscopic instruments and provides an anchor for the Valve Component. The Boot Assembly which maintains the pneumoperitoneum established for the surgical procedure. 2. PneumoLiner The PneumoLiner, has two sub-components: An Introducer for delivery of the PneumoLiner. The PneumoLiner, that when under pneumoperitoneum provides a space for morcellation of the chosen tissue that can be visualized. · Class 2

IT WAS REPORTED TO OLYMPUS BY THE CUSTOMER, PNEUMOLINER CONTAINMENT DEVICE FOR LAPAROSCOPIC MORCELLATION WITH LAPAROSCOPIC INSTRUMENT PORT WAS USED DURING A LAPAROSCOPIC SUPRACERVICAL HYSTERECTOMY (LSH) , ON A FEMALE PATIENT. THE USER REPORTED UPON REMOVAL THE PNEUMOLINER CONTAINMENT DEVICE, THE PATIENT SUSTAINED A SMALL ABRASION/NICK TO THE MESENTERY. THE PATIENT REQUIRED THREE VERY SMALL STITCHES TO MINIMIZE THE RISK OF CAPTURING A BRANCHING BLOOD VESSEL AND CUTTING OFF BLOOD SUPPLY TO THE MESENTERY. THE SMALL BOWEL REMAINED INTACT AND DID NOT REQUIRE SUTURES.

ON (B)(6) 2017, ASC RECEIVED A COMPLAINT THAT THE PNEUMOLINER (PL) CONTAINMENT DEVICE RIPPED OR CAME APART DURING A LAPAROSCOPIC SUPRACERVICAL HYSTERECTOMY. THE PHYSICIAN CONTINUED THE POWER MORCELLATION WITH THE RIPPED PL IN USE. THE CS INFORMED ASC THAT THE SURGEON DID NOT REMOVE THE TROCARS AS PER THE TRAINING PRIOR TO DEFLATION OF THE ABDOMEN. THEREFORE, THE PL DID NOT INFLATE FULLY. WARNING NUMBER 6 IN THE IFU STATES: "THE PNEUMOLINER MUST BE FULLY INFLATED (12 - 15MMHG) TO MINIMIZE THE RISK OF DAMAGE TO THE BAG AND ADJACENT ORGANS DURING MORCELLATION." THE CS NOTED A HOLE IN THE PL DURING THE CASE. THE SURGEON ATTEMPTED TO REMOVE THE PL BY PULLING ON THE PL WITH A LARGE AMOUNT OF FORCE. AS THERE WAS STILL TOO MUCH TISSUE REMAINING IN THE PL TO EXIT THE INCISION, THE TEAR/HOLE IN THE PL WAS PROPAGATED DUE TO THE LARGE FORCE EXERTED BY THE SURGEON. THIS RESULTED IN THE REMAINING CONTENTS OF THE PL SPILLING INTO THE PATIENT'S ABDOMEN AND THE PL EXITING THE INCISION. ASC SPOKE WITH SURGEON ON (B)(6) 2017. THE SURGEON INFORMED ASC HE HAD SOME DIFFICULTY GETTING THE PL TO UNROLL. HE STATED THERE WAS PLENTY OF SPACE TO MORCELLATE BUT HE WAS UNCERTAIN IF IT WAS FULLY DEPLOYED. HE WAS
