  • InjurySep 24, 2026Adverse Event Without Identified Device or Use Problem

This complaint consists of THT Registry data from Japan. The information was obtained through the registry, no further details have become available for this event. It was reported that on (b)(6) 2026, a 25mm Navitor Vision valve was selected for implant using a small FlexNav Delivery System (DS). During the preparation, loading was not successful as while passing the valve through the DS, the tip of the valve does not expand uniformly, causing the struts to overlap. After multiple attempts, this situation remained unchanged so a new 25mm, Navitor Vision was replaced resulting in successful implantation without any complications. The patient was in a stable condition. It was reported through the THT registry from Japan that on (b)(6) 2026, a 25mm Navitor Vision Valve was implanted. On (b)(6) 2026, the patient experienced severe conduction disturbance and required a pacemaker implant.

It was reported that on (b)(6) 2026, a 25mm Navitor Vision valve was selected for implant using a large FlexNav Delivery System (DS). The patient did not have a history of Right Bundle Branch Block (RBBB), Left Bundle Branch Block (LBBB) and 1st/2nd Atrioventricular Block (AV) block. The patient has a severely calcified leaflets with calcification extending in the Left Ventricular Outflow Tract (LVOT). The patient's aortic valve angle was 52 degrees. Pre-implant Balloon Aortic Valvuloplasty (pre-BAV) was performed by using a 20mm, non-Abbott balloon. During the procedure, while advancement of the FlexNav DS across the annulus, the patient developed with a complete Atrioventricular (AV) block. A temporary pacemaker was placed to stabilize the cardiac rhythm. At 80 percent deployment, an incomplete stent opening (ISO) was observed then the valve was recaptured. On the second attempt, the valve was placed at a depth of 4mm on the non-coronary cusp (NCC), ISO remained unchanged; however, it was full deployed in the native annulus at a depth of 4mm on the non-coronary cusp (NCC) and ISO was observed to be less than prior to deployment. It was reported that on (b)(6) 2026, a 25mm Navitor Vision valve was selected for implant using a FlexNav Delivery System (DS). The patient's aortic valve angle was measured to be 54 degrees. Pre-implant Balloon Aortic Valvuloplasty (pre-BAV) was performed by using a 23mm, non-Abbott balloon. During the procedure, starting from stent inflow of the valve being position at the level of the non-coronary cusp (NCC) pigtail. Following inflow edge of Navitor valve 3mm below the pig tail; upon release, the valve had migrated 7mm into the ventricle. No second valve was required to be implanted. The patient remained hemodynamically stable throughout the procedure and there was no clinically significant delay reported. The implanter believed the cause of migration to be due to the valve didn't have enough radial force. The patient was reportedly discharged.

  • MalfunctionSep 17, 2026Difficult to Fold, Unfold or Collapse

It was reported that on (b)(6) 2026, a 25 mm Navitor Vision Valve was selected for implant using a Small FlexNav Delivery System. During the procedure, it was reported that the valve was implanted too high at a final implant depth of 1 mm on the non-coronary cusp. A post-balloon aortic valvuloplasty was performed using a 22 mm non-Abbott balloon to address and resolve valve under expansion. It was reported that there were concerns that the valve would embolize. As a result, a second Small FlexNav Delivery System was opened in preparation for implantation of a second valve; however, a second valve was ultimately not required as the valve did not embolize. The delivery system was not used and was reportedly discarded at the site. The patient did not have any clinical signs or symptoms or experience any issues or adverse events during or after the procedure. There was no clinically significant delay, and the patient remained hemodynamically stable throughout the procedure. The user believed the valve expanded non-uniformly due to excess calcium. The patient's status was reported as stable and discharged.

(b)(6) - VISTA Nova, Patient Site ID: (b)(6). It was reported that on 03 August 2026, an unknown Navitor was selected for implant. The length of the membranous septum measured 5.3mm. There was no calcification extending beneath the aortic annular plane in the interventricular septum. While in the Cusp Overlap View (COV), the inflow edge of the constrained valve within the capsule was positioned at the base of aortic annulus while the pigtail positioned was confirmed to be at the bottom of the non-coronary cusp (NCC). Approximately one week post-procedure, it was then reported that asymptomatic intermittent atrioventricular block was observed. Sinus rhythm was restored and a leadless pacemaker was implanted on 17 August 2026 as a backup measure due to upcoming scheduled non-cardiac surgery and risk of recurrent heart block. Prolonged hospitalization was reported for monitoring of cardiac rhythm. The patient status was reported discharged.

  • DeathSep 11, 2026Migration or Expulsion of Device; Off-Label Use

It was reported that on (b)(6) 2026, a 25mm Navitor Valve with a Small FlexNav Delivery System were chosen for Off Label transcatheter aortic valve-in-valve procedure of a previously implanted 25mm Epic Plus Valve. During post-dilatation of the 25mm Navitor Valve with a 24mm non-Abbott balloon, it was noted there was a complication involving fracture of the 25mm Epic Plus Valve, and the 25mm Navitor Valve had migrated in the aortic direction, causing partial right and left coronary artery obstruction. The migrated valve was snared and repositioned to allow coronary access and placement of a new valve. Subsequently, a decision was made to prepare a second 25mm Navitor Valve with a second new Small FlexNav Delivery System that was successfully implanted within the 25mm Epic Plus Valve. The patient remained hemodynamically stable throughout the procedure. Post-operatively, patient suffered an ischemic stroke, which worsened over time, and ultimately leading to patient death on (b)(6) 2026.

It was reported that on (b)(6) 2026, a 25 mm Navitor Vision Valve was selected for a valve-in-valve procedure. The Navitor Valve was implanted within a 25 mm non-Abbott surgical aortic valve that had been implanted in 2015. No pre-dilation was performed. The patient's aortic valve angle was reported as 43 degrees. During the procedure, the implant depth at approximately 80% deployment was reported as 4 mm at the non-coronary cusp (NCC) and 4 mm at the left coronary cusp (LCC). The Navitor valve was successfully released; however, upon release, the non-marker side of the Navitor valve moved upward. Immediately prior to migration, the implant depth of the Navitor valve was reported as 2 mm below the 25 mm non-Abbott valve frame. It was clarified that the valve moved superiorly toward the aortic arch. The final implant depth after migration was reported as 0.0 mm at the non-coronary cusp (NCC) and 2.0 mm at the left coronary cusp (LCC). Moderate to severe paravalvular leak (PVL) was observed. The implanter checked for non-uniform expansion (NUE), and no NUE was observed. No eccentric calcification or anatomical/tissue-calcium interference affecting valve expansion was reported. Post-d
