MAUDE report 50923782 ↗InjurySep 24, 2026Difficult to Fold, Unfold or Collapse; Incomplete Coaptation; Intravalvular regurgitation
It was reported that on (b)(6) 2026, a 23mm Navitor Vision valve was selected for implant using a small FlexNav Delivery System (DS). Pre-implant Balloon Aortic Valvuloplasty (pre-BAV) was performed by using a 20mm, non-Abbott balloon. During the procedure, an incomplete stent opening (ISO) was observed but able to be mitigated as per the established steps. The valve was able to be implanted successfully without complications at a depth of 3mm on the non-coronary cusp (NCC). Post-implant, mild paravalvular leak (PVL) was noted. Post-implant Balloon Aortic Valvuloplasty (post-BAV) was performed by using a 23mm, non-Abbott balloon with an additional 2ml inflation to optimize prosthesis expansion and correct the PVL. Post-procedure, Transthoracic echocardiographic (TTE) evaluation showed significant central aortic regurgitation, confirming prosthetic valve dysfunction with reported leaflet inactivity and hemodynamic instability characterized by alternating hypertension and hypotension. A second 23mm, Navitor Vision valve was implanted within the index valve. Final echocardiogram assessment showed absence of central regurgitation and PVL. No clinically significant delay occurred, no pr

(b)(4) VISTA Nova, Patient Site ID: (b)(6).It was reported that on (b)(6) 2026, a 23mm Navitor with Radiopaque Markers was chosen for implant. The length of the membranous septum was 4.6mm. There was no calcification extending beneath the aortic annular plane in the interventricular septum. While in the Cusp Overlap View (COV), the inflow edge of the constrained valve within the capsule was positioned at the base of aortic annulus while the pigtail positioned was confirmed to be at the bottom of the non-coronary cusp (NCC). The final implant depth from the NCC to the ventricular end of the frame was 3.0mm. It was reported post-procedure, patient experienced complete heart block on (b)(6) 2026. A decision was made to continue temporary pacing while monitoring the clinical course. Prolonged hospitalization was reported due to monitoring of rhythm before discharge on (b)(6) 2026. As heart block persisted, a permanent pacemaker was implanted on (b)(6) 2026.

It was reported that on (b)(6) 2026, a 23 Navitor Vision valve was selected for implant using a small FlexNav Delivery System (DS) due to the patient's anatomy with severely calcified aortic valve with tapered Left Ventricular Outflow Tract (LVOT) and smaller Sinus of Valsalva (SOV). Pre-implant Balloon Aortic Valvuloplasty (pre-BAV) was performed by using an 18mm, non-Abbott balloon. The valve was loaded and check with X-Ray. During the procedure, the physicians used rotation move to make insertion swifter and some resistance was experienced with the FlexNav DS. At 80 percent, the valve appeared to be constrained therefore a partial resheath was performed and it was observed that the nose cone was not recaptured completely. On the second attempt, the valve expanded comparatively better but significant paravalvular leak (PVL) was observed then the physician's deemed that the 23mm valve to be undersized and a second 25mm, Navitor Vision valve was decided to be replaced using a new small FlexNav DS. Once the loading was completed, 23mm, Navitor Vision valve was attempted for recapture but once again the retraction issue persisted while the nose cone being exposed. DS was retracted in

It was reported that on (b)(6) 2026, a 23 mm Navitor Vision Valve was selected for implant using a Small FlexNav Delivery System. The length of the membranous septum was reported to be 1.9 mm. No calcification was confirmed from the annulus to the subvalvular left ventricular outflow tract (LVOT). During the procedure, complete atrioventricular block (CAVB) developed following deployment of the valve. A temporary pacemaker was subsequently placed, and the patient was transferred from the procedure room with the temporary pacemaker in place. The final implant depth of the Navitor valve was reported as 3 mm at the non-coronary cusp (NCC) and 5 mm at the left coronary cusp (LCC). The patient remained hemodynamically stable throughout the procedure and there was no clinically significant delay. Post-operative monitoring was performed. The patient recovered from CAVB the following day, however; CAVB recurred and a permanent pacemaker was subsequently implanted. As the patient had a short membranous septum (MS), the implanter believed that the event was related to either wire manipulation during the procedure or the position of the valve. The patient's status at the time of reporting was

During the procedure, an incomplete stent opening (ISO) was observed and a recapture was performed and the ISO was reportedly mitigated. The valve was able to be implanted successfully at a depth of 6mm both on the non-coronary cusp (NCC) and left-coronary cusp (LCC). The patient remained hemodynamically stable throughout the procedure and there was no clinically significant delay reported. Post-procedure less than 48 hours, moderate paravalvular leak (PVL) was observed. Post-implant Balloon Aortic Valvuloplasty (post-BAV) was performed by using a 20mm, non-Abbott balloon. Moderate PVL remained unchanged. The user believes the non-uniform expansion was caused by calcification. The patient's condition is unknown.

It was reported that on (b)(6) 2026, a 23mm Navitor Vision valve was selected for implant using a small FlexNav Delivery System (DS). The patient has a prior medication history of 30 mg, Lixiana® (edoxaban). Pre-implant Balloon Aortic Valvuloplasty (pre-BAV) was performed by using a 23mm, non-Abbott balloon. Upon insertion of 260cm, spring wire (non-Abbott brand, J-tip) into the left ventricle, the patient's blood pressure became low, and an unclear Percutaneous Cardiopulmonary Support (PCPS) was performed. During the procedure, the valve was able to be implanted. Post-implant, aortic root rupture and a type B dissection had occurred; small amount of circumferential pericardial effusion was observed to be at the apex of heart. Pericardiocentesis was attempted to resolve the event. However, the procedure was aborted because the amount of pericardial effusion was minimal, and further puncture was considered too risky. ECMO (Extracorporeal Membrane Oxygenation) was subsequently initiated. The physician(s) concluded the presence of cardiac tamponade. The user believed the cause of pericardial effusion to be due to the 23mm, non-Abbott balloon. ECMO has been discontinued; however, the p
