  • MalfunctionAug 13, 2026Premature Discharge of Battery; Premature Elective Replacement Indicator; Battery Problem: High Impedance

Additional information received. The patients battery status has rebounded. Confirmed by session report. The battery rebound event was confirmed to be related to a known event cause by the impedance behaviors in the beginning of the battery's life. Internal investigation identified that in some cases, pulse generators reach the 25% battery indicator earlier than expected and later rebound without any significant programming changes. This behavior was determined to be due to an increased duration of high battery impedance during generator battery beginning-of-life. There is no evidence of a device malfunction. No other relevant information has been received to date.

  • DeathJul 17, 2026Adverse Event Without Identified Device or Use Problem

IT WAS REPORTED THAT A VNS PATIENT WITH SEVERE EPILEPSY SECONDARY TO PITT-HOPKINS SYNDROME DIED ON (B)(6) 2026 FROM ASPIRATION-RELATED LUNG DISEASE/PNEUMONIA. THE PATIENT HAD A LONG HISTORY OF NEUROLOGICAL IMPAIRMENT AND CHRONIC DYSPHAGIA THAT PREDATED VNS THERAPY. VNS THERAPY WAS INITIATED IN 2017, WITH GENERATOR REPLACEMENTS IN 2021 AND (B)(6) 2026 DUE TO NORMAL BATTERY DEPLETION. PRIOR TO THE (B)(6) 2026 GENERATOR REPLACEMENT, THE GENERATOR HAD BEEN INACTIVE FOR APPROXIMATELY SIX MONTHS BECAUSE OF BATTERY DEPLETION. DURING THIS PERIOD, THE PATIENT'S SWALLOWING DIFFICULTIES AND COUGHING HAD ALREADY WORSENED. SETTINGS WERE ADJUSTED FOR DYSPHAGIA. IN (B)(6) 2026, THE PATIENT DEVELOPED SEVERE ASPIRATION-RELATED PNEUMONIA REQUIRING ICU ADMISSION AND SUBSEQUENTLY DIED. ALTHOUGH CLINICIANS RECOMMENDED TEMPORARILY SUSPENDING VNS STIMULATION USING THE MAGNET TO EVALUATE A POSSIBLE RELATIONSHIP BETWEEN STIMULATION AND COUGHING, THE FAMILY DECLINED. THE TREATING PHYSICIAN CONCLUDED THAT THE PATIENT'S DEATH WAS MORE LIKELY RELATED TO PROGRESSION OF PRE-EXISTING DYSPHAGIA AND ASPIRATION RISK, POTENTIALLY EXACERBATED BY INTUBATION ASSOCIATED WITH THE (B)(6) 2026 GENERATOR REPLACEMENT PROCEDUR

THE GENERATOR AND LEAD PASSED ALL SPECIFICATIONS PRIOR TO DISTRIBUTION. THE GENERATOR AND LEAD WERE HP STERILIZED. LATER IT WAS CONFIRMED THAT THE DEVICES WERE EXPLANTED. IT WAS REPORTED THAT THE EXPLANTED DEVICES WILL NOT BE AVAILABLE FOR RETURN TO THE MANUFACTURER DUE TO HOSPITAL PROTOCOL. • InjuryJun 24, 2026Adverse Event Without Identified Device or Use Problem

A NURSE REPORTED THAT IT IS SUSPECTED THAT THE VNS CAUSED HIGH GRADE AV BLOCK AND BRADYCARDIA TO 24 BPM FOR THE PATIENT. ON (B)(6) 2025, THE PATIENT EXPERIENCED EPISODES OF GOING PALE, SLURRING SPEECH, FEELING SICK AND UNSTEADY ON FEET. THIS CAUSED THE PATIENT TO SLEEP FOR SEVERAL HOURS AFTERWARDS. FURTHER DETAILS RECEIVED NOTING THAT THE PATIENT ALSO EXPERIENCED SYNCOPE BUT THERE WERE NO NOTED TRAUMATIC EVENTS OR TRIGGERS PRIOR TO THE ARRHYTHMIA. THE EVENT DID NOT OCCUR FOLLOWING A MEDICATION CHANGE, IT DID NOT CORRELATE WITH THE ON TIME AND DID NOT OCCUR WHILE PERFORMING A DIAGNOSTIC OR FOLLOWING A SETTINGS CHANGE. AN ECG WAS PERFORMED TO DIAGNOSE THE ARRHYTHMIA AND IT WAS ASSESSED AS BEING RELATED TO VNS THERAPY STIMULATION. THE PATIENT'S DEVICE WAS DISABLED ON (B)(6) 2025 AND THERE WAS NO FURTHER AV BLOCK OR BRADYCARDIA SEEN ON THE HOLTER AND ZIO PATCH RESULTS. NO OTHER RELEVANT INFORMATION HAS BEEN RECEIVED TO DATE.

ADDITIONAL INFORMATION RECEIVED NOTING THAT THE INFECTION FIRST PRESENTED ON (B)(6) 2026 AND THE PATIENT UNDERWENT EXPLANT SURGERY ON (B)(6) 2026. THE CAUSE OF THE INFECTION WAS DUE TO THE PATIENT HAVING A REACTION TO THE SUTURES FROM THE VNS REPLACEMENT. THE DEVICE HISTORY RECORDS FOR THE GENERATOR WERE REVIEWED. THE GENERATOR WAS CONFIRMED TO HAVE BEEN HP STERILIZED PRIOR TO DISTRIBUTION INTO THE FIELD. THE DEVICE PASSED ALL SPECIFICATIONS, INCLUDING QUALITY CONTROL INSPECTION, AND SHOWED NO NON-CONFORMITIES, PRIOR TO ITS RELEASE INTO THE FIELD FOR DISTRIBUTION. DEVICE EVALUATION IS NOT NECESSARY BECAUSE THE REPORTED EVENT HAS BEEN DETERMINED AS NOT RELATED TO THE FUNCTIONALITY OR DELIVERY OF THERAPY OF THE DEVICE.
