  • MalfunctionAug 31, 2026Detachment of Device or Device Component

It was reported to Boston Scientific Corporation that, during the use of a Capio device, the bullet needle detached from the suture when the device was removed from the cavity during the first pass through the ligament. An attempt was made to recover the needle, but it could not be found because it was too small to locate. The procedure was completed using the original Capio device. There were no patient complications reported.

INITIALLY, THE BULLET WAS THOUGHT TO BE LODGED IN THE PELVIS; HOWEVER, IT WAS LATER DETERMINED TO BE RETAINED WITHIN THE CHANNEL ON THE CAPTURE SIDE OF THE DEVICE. THE PROCEDURE WAS COMPLETED USING ANOTHER CAPIO DEVICE. THERE WERE NO PATIENT COMPLICATIONS REPORTED. NOTE: THIS MANUFACTURER REPORT PERTAINS TO THE SECOND OF TWO DEVICES USED DURING THE PROCEDURE.
