Quick summary

This Boston Scientific Flexiva Pulse laser fiber is a single-use fiber used with a powered laser console, mainly for breaking kidney stones. The 28 reports mostly describe the fiber tip snapping or burning off, sometimes leaving a loose piece inside the patient, plus connectors overheating and fibers failing to fire. No deaths and 5 injuries. Two open recalls from May 2023 say a wrong internal component can cut power and let the connector overheat enough to burn someone touching it.
