  IT WAS REPORTED THAT A PATIENT UNDERWENT AN UNKNOWN PROCEDURE ON AN UNKNOWN DATE AND SUTURE WAS USED. THE PATIENT HAS A RETAINED NEEDLE IN THE ABDOMEN. ADDITIONAL INFORMATION WAS REQUESTED.

  IT WAS REPORTED THAT A PATIENT UNDERWENT A PANCREATICODUODENECTOMY PROCEDURE ON (B)(6) 2018 AND A SUTURE WAS USED. DURING SURGERY, IT WAS FOUND THAT THE NEEDLE TIP HAD BEEN MISSING BEFORE USE. UPON EVALUATION OF THE RETURNED SAMPLE, A FRACTURE WAS OBSERVED AT THE TIP OF THE NEEDLE. THERE WERE NO ADVERSE PATIENT CONSEQUENCES REPORTED. NO ADDITIONAL INFORMATION WAS PROVIDED.
