  IT WAS REPORTED DURING INSERTION ON THE LEFT JUGULAR, THE GUIDEWIRE DID NOT GO THROUGH THE ARROW RAULERSON SYRINGE (ARS). NO PATIENT HARM REPORTED. A NEW SET WAS USED ON THE LEFT SUBCLAVIAN VEIN SUCCESSFULLY. THE PATIENT'S CONDITION IS REPORTED AS FINE.
