  ACCORDING TO THE REPORTER, DURING AN OPEN CREATION OF THE ARTERIOVENOUS FISTULA, WHILE TYING THE VESSEL, THE THREAD BROKE IN THE MIDDLE WHILE DOING A HAND TYING TECHNIQUE. THE STRAND FELT SO SMALL AND WAS EASY TO BREAK. A NEW SUTURE WAS USED TO RESOLVE THE ISSUE. NO PATIENT INJURY.
