ACCORDING TO THE REPORTER, THE (HEMODIALYSIS) HD CATHETER WAS DISPLACED 10 CM OUT OF THE VESSEL (30 CM LINE). WHEN THEY CAME TO BEDSIDE, THE CLEAR PLASTIC PIECE THAT PERMITS SUTURE OF THE CATHETER TO THE SKIN WAS STILL IN PLACE WITH SUTURES INTACT, BUT THE CATHETER HAD SEPARATED FROM IT AND WAS FREE SLIDING (HENCE THE DISPLACEMENT).
