  ACCORDING TO THE REPORTER, DURING AN OPEN PROCEDURE, WHILE SUTURING THE VAGINA AFTER BIRTH, THE DEVICE'S THREAD GOT LOOSE. A NEW THREAD FROM THE SAME BOX WAS USED TO COMPLETE THE CASE. THERE WAS NO PATIENT INJURY."
