  ACCORDING TO THE REPORTER, DURING PROCEDURE, THE STYLET WOULD NOT PASS THROUGH THE LUMEN. STYLETS WERE THREADED WITH THE CORRECT ORIENTATION, BUT SOMETHING IN ONE OF THE LUMENS PREVENTED THE STYLET FROM THREADING COMPLETELY THROUGH THE LUMEN FROM ONE END TO THE OTHER - OCCLUSION WAS NOTED. NOTHING UNUSUAL OBSERVED DURING VISUAL INSPECTION. NO DAMAGE NOTED ON THE DEVICE PACKAGING. THE BOX THAT THE DEVICE CAME IN WAS INTACT. THE CATHETER WAS NOT REPAIRED AND HAD NO LEAK. TEGO WAS NOT UTILIZED. THERE WAS NO LUER ADAPTER ISSUE. THE STYLET WAS PART OF THE KIT BEING USED. NO OTHER PRODUCTS BEING UTILIZED WITH THE DEVICE. THERE WAS NO BLOOD LOSS AND NO BLOOD TRANSFUSION WAS REQUIRED. NO MEDICAL INTERVENTION/TREATMENT PROVIDED DUE TO THE EVENT. IT WAS SAID THAT THE PROCEDURE WAS PERFORMED AND COMPLETED WITH ANOTHER CATHETER (THEY PULLED OUT ANOTHER ONE). THERE WAS NO REPORTED PATIENT INJURY.
