  • MalfunctionJan 22, 2025Detachment of Device or Device Component; Detachment of Device or Device Component

THE CUSTOMER REPORTED THE PATIENT HAD A WEIGHTED STYLET 8 FRENCH NASOGASTRIC TUBE (NGT) PLACED AND CONFIRMED BY CHEST X-RAY (CXR). WHEN CHANGING THE NOSE SECUREMENT, IT WAS NOTICED THAT THE EXTERNAL LENGTH TICK MARK AT THE NARE HAD A DISCREPANCY OF 8CM. PLACEMENT LENGTH WAS 57CM, PREVIOUS SHIFTS HAD DOCUMENTED 60CM, BUT NURSE CONFIRMED LENGTH OF 65CM. IMAGING WAS COMPLETED, SHOWING THAT THE WEIGHTED TIP AND TUBING HAD BECOME DISCONNECTED. THE NURSE WAS ADVISED TO RETRACT THE NGT BY 5CM (FROM 65CM TO 60CM) AND THEN DO A REPEAT KIDNEY, URETER, AND BLADDER (KUB). THE 2ND KUB CONFIRMED THAT THE WEIGHTED TIP AND TUBING HAD INDEED COME APART, AND THE WEIGHTED TIP WAS FLOATING FREELY IN THE STOMACH. IT WAS EXPECTED THAT THE PATIENT WOULD PASS THE TIP. PER ADDITIONAL INFORMATION PROVIDED BY THE REPORTER ON 23JAN2025, THE DEVICE WAS REPLACED WITH A NEW ONE. THE PATIENT WAS EXPECTED TO PASS THE TIP, BUT PASSED AWAY DUE TO HIS CRITICAL UNDERLYING ILLNESS; THE DEVICE RETENTION WAS NOT RELATED TO THE PATIENT'S DEATH OR WORSENING OF HIS CONDITION.

  • InjuryJan 21, 2025Detachment of Device or Device Component; Detachment of Device or Device Component

THE CUSTOMER REPORTED THE NURSE REMOVED THE PATIENT'S NASOGASTRIC TUBE AND THE WEIGHTED TIP WAS NOT ON THE END. THE NASOGASTRIC TUBE DID NOT HAVE RESISTANCE WHEN REMOVED. THE PATIENT COUGHED AFTER THE REMOVAL AND STATED, "I FEEL LIKE THERE IS SOMETHING IN MY THROAT". THE NURSE EXAMINED THE NASOGASTRIC TUBE AND THE WEIGHTED TIP WAS NOT PRESENT. THE TUBE APPEARED TO BE FRAYED WHERE IT WAS BROKEN. THE PATIENT WAS MONITORED APPROPRIATELY AND WAS NEVER IN RESPIRATORY DISTRESS, THOUGH IMAGING INDICATED THE TIP TO BE IN THE BRONCHUS, REQUIRING A BRONCHOSCOPY TO REMOVE.

  • InjuryOct 29, 2024Material Separation; Material Separation

THE CUSTOMER REPORTED THE TIP BROKE OFF WHILE IN THE PATIENT'S STOMACH. PER ADDITIONAL INFORMATION PROVIDED ON 29OCT2024, THE TUBING ITSELF SPLIT INTO TWO BUT IT LOOKS TO BE NEAR THE WEIGHTED TIP. THEY BELIEVE THAT THE TUBE WAS PARTIALLY BROKEN/FRAYED AND AS THE RN WENT TO PULL IT OUT (THE PATIENT HAD PASSED HIS SWALLOW EVAL) THE WEIGHT OF THE TIP CAUSED IT TO FULLY BREAK OFF. IT THEN, AS IT CAME OUT OF THE ESOPHAGUS, FELL INTO THE BRONCHUS. THE PATIENT HAD TO HAVE A BRONCH TO REMOVE THE FOREIGN BODY. A NEW FEEDING TUBE DID NOT NEED TO BE PLACED AS THE PATIENT WAS ABLE TO NOW TAKE IN ORAL NUTRITION.
