ACCORDING TO THE REPORTER, PRE-OPERATIVELY, WHEN THE PACKAGE WAS OPENED, THE CONNECTION BETWEEN THE THREAD AND THE NEEDLE WAS FOUND TO BE APART FROM THE BEGINNING. ANOTHER DEVICE FROM ANOTHER MANUFACTURER WAS USED. THERE WAS NO PATIENT INVOLVEMENT.
