ACCORDING TO THE REPORTER, DURING A CORONARY ARTERY BYPASS GRAFT, WHEN OPENING THE PACKAGE, THE NEEDLE AND THREAD WERE FOUND TO BE DISENGAGED. AN X-RAY WAS DONE TO RESOLVE THE ISSUE.
