  ACCORDING TO THE REPORTER, DURING A PROCEDURE, AT THE FIRST PACKET, THE PRODUCT WAS EASILY DETACHED. THOUGH 3 PACKETS WERE USED, MOST OF THEM WERE DETACHED MORE EASILY THAN USUAL. THE PROCEDURE WAS COMPLETED WITH ANOTHER DEVICE. THERE WAS NO PATIENT INJURY.

  ACCORDING TO THE REPORTER, DURING A PROCEDURE, PRODUCT WAS USED AT CLOSURE OF THE ABDOMINAL CAVITY. WHEN LIGATING, THE THREAD BROKE EASILY. THE PRODUCT WAS USED IN THE USUAL WAY AND FELT THAT THE THREAD STRENGTH WAS WEAK. ANOTHER PACKET WAS USED AND THE OPERATION WAS COMPLETED. THERE WAS NO PATIENT INJURY.
