  • MalfunctionAug 27, 2026Fluid/Blood Leak

It was reported that during IV catheter insertion, after the needle was inserted and the catheter advanced, blood continued to flow into the flashback chamber, along the sides of the device, and into the needle hub. Following successful IV insertion with positive flashback, the IV device was removed from the catheter; however, blood continued to flow from the catheter in the patient's extremity, indicating that the blood control feature may not have functioned as intended. The issue was observed by using IV catheters from the same box. A catheter from a different box (different lot number) was subsequently used, and the blood control feature functioned as expected, with no blood flow observed from the catheter after removal of the IV device and without distal pressure applied. There was patient involvement and no patient harm.
