We found 3 models of TACTRA by Boston Scientific Corporation in the FDA's device registry (GUDID). Together they are named in 260 adverse event reports in the FDA's MAUDE database, received between Aug 22, 2019 and Aug 31, 2026 — classified as 240 injuries, 20 malfunctions.
The most frequently reported specific device problem is “Insufficient Device Problem Information” (53 reports).
Reports come from manufacturers, hospitals and patients and are not verified by the FDA. A report does not prove the device caused the event. Widely used products collect more reports, so totals can't be used to compare products or estimate failure rates. Only reports that name a model's exact device identifier are counted. InjuryAug 31, 2026Malposition of Device
It was reported that a patient implanted with a Tactra penile prosthesis experienced glans penile pain, loss of penile length, and glans instability. The patient also reported that the prosthesis was malpositioned relative to the glans. A penile ultrasound was performed and confirmed prosthesis malposition within the glans and glans instability, with the prosthesis noted to be poorly positioned in the glans. A revision procedure was scheduled to reconstruct the corpora cavernosa and replace the penile prosthesis. Subsequently, the revision surgery was performed, during which the Tactra penile prosthesis was explanted and replaced with an inflatable penile prosthesis (IPP). No additional patient complications were reported.

It was reported that the patient with this Tactra malleable penile prosthesis experienced axial instability of the device, difficulty achieving satisfactory penetration, and an unnatural feeling. Imaging did not identify suspected cross-over or fracture of the device. The patient also reported pain; however, the physician indicated that the pain was not specifically related to the prosthesis and was associated with the patient's prior clinical condition. Several days later, the device was explanted and replaced. No additional information was provided.
