MalfunctionApr 29, 2026Communication or Transmission Problem
THE END USER REPORTED UPON ARRIVAL AT A PATIENT'S HOME, THE FASTENING SYSTEM INSTALLED IN THE AMBULANCE WOULD NOT ALLOW THE STRETCHER TO BE UNLOADED FROM THE AMBULANCE. THE END USER CONFIRMED THE FASTENING SYSTEM WAS OPERATING PROPERLY PRIOR TO ARRIVAL ON SCENE. THE END USER ADVISED THAT THE CREW ELECTED TO NOT TRANSITION TO MANUAL OPERATION OF THE SYSTEM BUT RATHER THEY CALLED FOR A BACKUP AMBULANCE TO COMPLETE THE PATIENT TRANSFER. THIS RESULTED IN A REPORTED 30 MIN DELAY. THE PATIENT'S CONDITION WAS REPORTED AS "HAVING TROUBLE BREATHING". THERE WERE NO ALLEGATIONS THAT THE DELAY RESULTED IN AN ADVERSE HEALTH CONSEQUENCE FOR THE PATIENT. A FOLLOW UP CALL TO THE END USER WAS MADE AND IT WAS DETERMINED THE FASTENING SYSTEM HAD BECOME UNPAIRED WITH THE STRETCHER WHICH RESULTED IN THE DISRUPTION OF THE COMMUNICATION NECESSARY FOR OPERATION OF THE POWERED FEATURES.

MAUDE report 25033098 ↗MalfunctionApr 29, 2026Communication or Transmission Problem
THE END USER REPORTED THE POWERED FASTENING SYSTEM WOULD NOT FULLY RETRACT THE CARRIAGE AND STRETCHER WHEN LOADING A PATIENT INTO THE AMBULANCE. THE MEDIC CREW TRANSITIONED INTO MANUAL OPERATION OF THE FASTENING SYSTEM TO COMPLETE THE TRANSFER TO THE HOSPITAL. UPON UNLOADING AT THE HOSPITAL, THE FASTENING SYSTEM WOULD THEN NOT RELEASE THE STRETCHER FROM THE FASTENER. THE PATIENT WAS LIFTED FROM THE STRETCHER AND ONTO A HOSPITAL STRETCHER THAT WAS BROUGHT OUT TO THE AMBULANCE. THERE WERE NO ALLEGATIONS OF ADVERSE HEALTH CONSEQUENCES TO THE PATIENT AS A RESULT OF THE DELAY IN LOADING AND UNLOADING. AFTER THE TRANSPORT, THE END USER WAS ABLE TO RELEASE THE STRETCHER FROM THE FASTENER AND REPAIR THE FASTENING SYSTEM AND STRETCHER.

MAUDE report 25033071 ↗DeathMar 25, 2026Mechanical Problem; Intermittent Communication Failure
THE END USER REPORTED AFTER ARRIVAL AT THE HOSPITAL WITH AN ACTIVE CARDIAC PATIENT; WHILE UNLOADING THE STRETCHER FROM THE AMBULANCE, THE LEGS OF THE STRETCHER HAD TO BE MANUALLY LOWERED BUT THEN THE CARRIAGE ARMS WOULD NOT LOWER DOWNWARD TO RELEASE THE STRETCHER FROM THE LOADER. THE CREW WAS ABLE TO MANUALLY LOWER THE CARRIAGE ARMS AND COMPLETE THE TRANSPORT INTO THE HOSPITAL. THE END USER ADVISED AN ESTIMATED TWO-MINUTE DELAY WAS EXPERIENCED DURING THE UNLOADING PROCESS. PATIENT DETAILS: MALE, APPROXIMATELY 220 LBS, PATIENT WAS EXPERIENCING AND BEING TREATED FOR CARDIAC ARREST ON SCENE AND THROUGHOUT THE TRANSPORT. CPR WAS BEING ADMINISTERED VIA LIFELINE ARM. THE PATIENT WAS LATER PRONOUNCED DECEASED AT THE HOSPITAL. THE END USER ADVISED THEY WERE UNABLE TO DUPLICATE THE ISSUE AFTER THE INCIDENT AND MADE THE DECISION TO KEEP THE POWER F2 IN SERVICE, IN MANUAL MODE ONLY, UNTIL THE EVALUATION. THIS CONTINUES TO BE AN ACTIVE INVESTIGATION AND ADDITIONAL INFORMATION WILL BE PROVIDED IN A FOLLOW UP REPORT.
