INFORMATION WAS RECEIVED FROM HEALTHCARE PROVIDER (HCP) VIA A MANUFACTURER REPRESENTATIVE REGARDING A PATIENT HAVING POSTERIOR SPINAL FUSION FOR SCOLIOSIS. IT WAS REPORTED THAT THE UNI-REDUCTION SCREWS HAVE TOGGLED, MAKING THE DEROTATION OF THE VERTEBRAL BODY AT T12 DIFFICULT/ IMPOSSIBLE TO MANIPULATE. UPON X-RAY, IT APPEAR THE SCREWS WERE NOT INLINE AND DID NOT LOOK LIKE UNI-AXIAL SCREWS. THE SCREWS WERE REMOVED AND UPON CHECKING, THE SCREWS HAVE MORE MULTIAXIAL TOGGLE THAN THERE SHOULD BE. IT APPEARED THAT THE SCREWS HAD BROKEN, MAKING THEM MORE MULTI-AXIAL THAN UNI-AXIAL THERE WAS NO PATIENT SYMPTOM REPORTED. THERE WERE NO FURTHER COMPLICATIONS REPORTED REGARDING THE EVENT.
