INFORMATION WAS RECEIVED FROM A HEALTHCARE PROVIDER (HCP) VIA MANUFACTURER REPRESENTATIVE REGARDING A PATIENT WHO UNDERWENT POSTERIOR LUMBAR FUSION SPINAL THERAPY FOR L1 COMPRESSION FRACTURE. IT WAS REPORTED THAT DURING FOLLOW-UP VISIT, THE LEFT SCREWS IN L2, 3, AND 4 WERE BACKED OUT WHICH WAS IDENTIFIED DURING THE IMAGING. THERE IS A PLAN OF REFIXATION. NO FURTHER COMPLICATIONS OR SYMPTOMS WERE REPORTED. ADDITIONAL INFORMATION WAS RECEIVED VIA MANUFACTURER REPRESENTATIVE THAT REVISION SURGERY WAS PERFORMED ON (B)(6) 2022 AND ALL THE REPORTED PRODUCTS WERE BACKED OUT. THE IMPLANT DATE IS (B)(6) 2022 AND EXPLANT DATE IS (B)(6) 2022.
