  • InjuryNov 14, 2023Difficult to Remove; Activation, Positioning or Separation Problem; Difficult to Remove; Activation, Positioning or Separation Problem

AN MVP WAS INTENDED TO BE USED. IT WAS REPORTED THE PRODUCT COULD NOT BE RELEASED IN THE VESSEL. THE PLUG COULD NOT BE REMOVED VIA THE SCREW THREAD IN THE VESSEL. SINCE THE PLUG WAS ALREADY OPENED IN THE VESSEL AND COULD NO LONGER BE FOLDED OVER THE NORMAL CATHETER, THE PLUG HAD TO BE RETRIEVED USING A SHEATH RETRACTION VIA THE SHEATH. THE WITHDRAWAL COULD BE CARRIED OUT WITHOUT ANY VASCULAR DAMAGE TO THE PATIENT. NO SERIOUS CONSEQUENCES FOR THE PATIENT, USER OR THIRD PARTY. THE INTERVENTION WAS DELAYED FOR A MOMENT BECAUSE THE PLUG HAD TO BE RECOVERED AND A NEW SYSTEM HAD TO BE PREPARED. THERE IS A POTENTIAL RISK THAT THE OPENED PLUG COULD INJURE A VESSEL WHEN WITHDRAWN. NO PATIENT INJURY.

Patient problems: No Clinical Signs, Symptoms or Conditions
