  MEDTRONIC RECEIVED INFORMATION THAT DURING USE OF A CUSTOM TUBING PACK, THE CUSTOMER REPORTED A LEAK OF THE ONE WAY VALVE. THE DEVICE WAS REPLACED TO COMPLETE THE PROCEDURE. THERE WAS NO PATIENT IMPACT ASSOCIATED WITH THIS EVENT. ADDITIONAL INFORMATION CONFIRMS THAT LESS THAN 1ML BLOOD WAS LOST, A BLOOD TRANSFUSION WAS NOT REQUIRED AND THERE ARE NO PATIENT INJURY AS A RESULT OF THE ISSUE.
