  MEDTRONIC RECEIVED INFORMATION THAT PRIOR TO USE OF A CUSTOM TUBING PACK, THE CUSTOMER REPORTED THAT AFTER SETTING UP THE EXTRACORPOREAL CIRCUIT, PRIMING, AND BEGINNING RECIRCULATION, THE MYOTHERM CARDIOPLEGIA HEAT EXCHANGER LEAKED PROFUSELY DURING PRESSURE TESTING AT 250 MMHG. THE DEVICE WAS REPLACED TO COMPLETE THE PROCEDURE. THERE WAS NO PATIENT INVOLVEMENT SO NO ADVERSE EFFECT OCCURRED. ADDITIONAL INFORMATION: THE PACK TRAY WAS UNDAMAGED, AND THE OUTSIDE BOX WAS NOT AVAILABLE.
