IT WAS REPORTED THAT AFTER PLACING AN OROGASTRIC TUBE FOR GASTRIC DECOMPRESSION, THE OROGASTRIC TUBE WAS VERIFIED WITH A KIDNEY, URETER, AND BLADDER X-RAY AND THEN HOOKED UP TO SUCTION AND WHISTLING WAS NOTED, AND THE SUCTION WAS NOT WORKING. UPON INSPECTION OF THE TUBE, THE OPENING BETWEEN THE CLEAR PART OF THE TUBE AND THE PIGTAIL WAS TORN IN TWO PLACES, CAUSING THE AIR FROM THE SUCTION TO PULL THROUGH THE HOLE IN THE TORN TUBE RATHER THAN FROM THE PATIENT'S STOMACH. THE TUBE HAD TO BE REMOVED, REPLACED, AND RE-VERIFIED.

IT WAS REPORTED THAT THERE WAS QUITE A DIFFERENCE IN THE OPENINGS. BOTH WERE 18FR., 48¿ NASOGASTRIC TUBES, BUT IT HAD DIFFERENT LOT NUMBERS. THE ONE WITH THE LARGER OPENING (LOT#NGHV0924) AND THE SMALLER OPENING (LOT#NGZI1386). THE FEEDBACK THAT GETTING WAS THAT THE ONE WITH THE LARGER OPENING WAS MAKING IT DIFFICULT TO ADVANCE. IT SUPPOSEDLY KEPT BENDING ON ITSELF DURING INSERTION. THE ONLY THING THAT THEY COULD THOUGHT OF THAT DUE TO LESS PLASTIC, MEANS LESS ENDURANCE WHEN CONFRONTED WITH MINIMAL RESISTANCE, WHICH RESULTS IN BENDING AND KINKING.

THE PATIENT REQUIRED ONE ATTEMPT FOR ET PLACEMENT AND ONE ATTEMPT FOR NGT PLACEMENT. PER THE ANESTHESIOLOGIST, THE TUBE WAS A TEMPORARY REPLACEMENT NGT FOR AN OUT OF STOCK ITEM. IT WAS REPORTED THE TUBE REQUIRES A DIFFERENT INSERTION TECHNIQUE BECAUSE IT IS NOT AS PLIABLE AND STAFF WERE UNAWARE OF THAT PROCESS. DURING THE SURGERY, A TRANSTHORACIC REDO HIATAL HERNIA REPAIR, AN ESOPHAGEAL PERFORATION WAS FOUND AND REPAIRED.
