Flow diverter may have a manufacturing issue related to the tantalum length and/or tantalum attachment pattern, which may lead the device to be incompletely open or not properly apposed to the vessel wall, which may necessitate device recapture and withdrawal of the delivery system and microcatheter together. Insufficient apposition could lead to long-term health consequences.

IT WAS REPORTED THAT THE FLOW DIVERTER STENT WAS USED TO TREAT A PREVIOUSLY TREATED RUPTURED POSTERIOR COMMUNICATING ARTERY (PCOM) ANEURYSM. THERE WAS A REMNANT FROM THE USE OF AN INTRALUMINAL FLOW DISRUPTION WEB DEVICE IN THIS RUPTURED PCOM. THE ANEURYSM LOCATION WAS AT DISTAL ICA. DISTAL MEASUREMENT OF THE ICA WAS 4.4MM AND PROXIMAL LANDING ZONE TO THE ANEURYSM MEASURED 3.8MM. THE SIZE OF STENT CHOSEN WAS A 5X15/9. PROPER PREP OF THE STENT WAS COMPLETED BEFORE ADVANCING INTO THE MICROCATHETER. THE DEVICE OPENED AND DEPLOYED NICELY WITH GOOD APPOSITION TO THE VESSEL WALL. REPORTEDLY, ABOUT 3 HOURS LATER, THE PHYSICIAN REPORTED THAT THE PATIENT HAD EXPERIENCED STROKE SYMPTOMS AND COULD NOT MOVE HALF OF THEIR BODY. THE PATIENT WAS TAKEN BACK TO THE ANGIO SUITE FOR IMAGING WHERE IT WAS DISCOVERED THAT THE STENT HAD MIGRATED INTO THE M1 SEGMENT AND HAD CLOT FORMATION INSIDE. THE PATIENT WAS GIVEN INTEGRALIN AND HAS IMPROVED. REPORTEDLY, THERE WAS SPASM EARLIER IN THE PROCEDURE. VERAPAMIL WAS GIVEN ABOUT 30-45 MINUTES BEFORE STENT WAS DEPLOYED. CURRENTLY THE PATIENT IS DOING A LITTLE BIT BETTER, BUT STILL HAD A STROKE AND IS WEAK ON THE LEFT SIDE. THE STENT WAS OVERSIZED AND WAS OPPOSE

WE RECAPTURED AND REMOVED DEVICE CHECKED AGAIN IN BOWL LOOKED LIKE IT WAS OPEN OK SO WE ATTEMPTED A SECOND ATTEMPT. UPON SECOND ATTEMPT TO DEPLOY DEVICE IT AGAIN OPENED DISTALLY BUT WOULD NOT OPEN IN THE MIDDLE. WE REMOVED DEVICE, SELECTED A DIFFERENT SIZE AND LENGTH OF DEVICE AND PREPPED AN INTRODUCED NEW FRED X. THE NEW DEVICE OPENED AND DEPLOYED JUST AS PLANNED. PATIENT WOKE UP POST PROCEDURE AND APPEARED TO HAVE MOBILITY OF ALL EXTREMATIES. POSSIBLE EFFASIA BUT WAS NOT COMPLETELY AWAKE AND FULLY ASSESSED AT THAT TIME. DESCRIPTION OF INTERVENTION: AFTER DEVICE DEPLOYMENT UPON DOING COMPLETION RUNS WE NOTICED CLOT IN THE M1/M2 SEGMENT. WE CROSSED BACK THROUGH THE FLOW DIVERTER WITH A HEADWAY 21 AND A SOFIA 5F 125 AND WENT AFTER THE CLOT 1ST PASS WITH TREVO STENT TREAVER OPEN UP M1/M2 BUT CLOT MOVED DISTAL WE MADE SECOND PASS WITH TREVO AND WERE ABLE TO RETRIEVE CLOT.
