(WEST IL), TO REPORT THAT UPON QUALITY CONTROL, 13 BOXES OF THE MP VIAL ADAPTER 13MM, LOT J075, WERE FOUND TO HAVE DAMAGED PRIMARY PACKAGING ON ALL INSPECTED SAMPLES. THE EVENT WAS DISCOVERED PRIOR TO PATIENT USE.
