IT WAS REPORTED DURING A PROCEDURE ON (B)(6) 2024 USING TRIALTIS THERE WAS DIFFICULTY USING THE INSTRUMENTS. DURING THE CASE, HE USED A 3-PIECE REDUCTION INSTRUMENT. AFTER THE ROD WAS SEATED AND THE SET SCREW WAS PLACED, THE OUTER SLEEVE OF THE INSTRUMENT WOULD NOT RELEASE FROM THE TOP OF THE SCREW. HE PULLED THE CENTER PORTION OF THE INSTRUMENT OUT COMPLETELY, BUT IT STILL WOULD NOT DISENGAGE. HE FIDDLED WITH THE CLIP AND USED ANOTHER INSTRUMENT TO TRY AND REACH AROUND TO THE BACK OF THE CLIP TO ASSIST IN RELEASING IT. HE WAS EVENTUALLY ABLE TO GET IT TO RELEASE, BUT IT TOOK ABOUT 3-1/2 MINUTES TO GET THE CLIP OFF OF THE TULIP. THE PROCEDURE WAS SUCCESSFULLY COMPLETED WITH A SURGICAL DELAY OF 3.5 MINS AND PATIENT OUTCOME WAS UNKNOWN.
