  • InjuryMay 5, 2026Fitting Problem

IT WAS REPORTED THAT THE VIDEOLARYNGOSCOPE WITH SIZE 3 BLADE WAS USED AND THE PATIENT WAS SUCCESSFULLY INTUBATED IN THE FIELD . WHEN THE HOSPITAL EXTUBATED THE PATIENT AFTER 5 DAYS IT WAS DISCOVERED THAT THE BLADE WAS STILL IN THE PATIENT'S THROAT. IT SEEMS THAT THE BLADE DID NOT LATCH ONTO THE HANDLE AS EXPECTED AND CAME OFF THE HANDLE DURING INTUBATION. IT WAS MADE SURE THAT THE BLADE WAS STILL INTACT AND NOT MISSING ANY PIECES WHEN REMOVED. THE PATIENT WAS ALIVE BUT AN IDENTIFIED INJURY WAS REPORTED.
