  IT WAS REPORTED THAT THERE WAS A NUB OR SMALL LUMP ON TRACH TUBE. THE DOCTOR HAD TO CUT THE NUB OR SMALL LUMP IN ORDER TO BE ABLE TO USE IT. THE NUB OR LUMP REPORTEDLY APPEARS TO BE AT THE SIX O'CLOCK POSITION ON THE TRACH. THERE WAS NO PATIENT HARM.
