  AS REPORTED TO ANGIODYNAMICS ON (B)(6) 2017: DURING PREP FOR AN ANGIOGRAPHIC PROCEDURE, WHEN THE CATHETER WAS REMOVED FORM THE STERILE PACKAGING, IT WAS NOTED THE TIP OF THE CATHETER HAD DETACHED INSIDE OF THE PACKAGING. THE DEVICE WAS SET ASIDE AND A NEW OF THE SAME WAS USED TO COMPLETE THE PROCEDURE. IT WAS REPORTED THE DEFECTIVE DISPOSABLE DEVICE IS AVAILABLE FOR RETURN TO THE MANUFACTURER FOR EVALUATION.
