MAUDE report 50570510 ↗InjuryAug 6, 2026False Negative Result
Summary: The customer ((b)(6) Mexico) reported a potential false negative Cryptococcus neoformans/gattii (C. neoformans/gattii) result on the FilmArray® Meningitis/Encephalitis (ME) Panel after testing a CSF sample from an 85-year-old male patient. The customer reported that due to the ME Panel result, that patient was indirectly impacted, specifically by a delay in treatment of approximately one day, which might lead to serious injury if it were to recur. No patient harm was reported, however it was reported the patient later expired for reasons unrelated to the ME Panel result. The investigation concluded that the most likely cause for the discrepant Cryptococcus results was comparator sensitivity/specificity differences between the ME Panel and traditional laboratory methods.

MAUDE report 50092758 ↗InjuryFeb 12, 2026False Negative Result
(B)(6). PUBLISHED A STUDY, "(B)(6)" WHICH DESCRIBES A POTENTIAL FALSE NEGATIVE HSV-1 AND A POTENTIAL FALSE NEGATIVE HHV-6 ON THE FILMARRAY MENINGITIS/ENCEPHALITIS (ME) PANEL DURING TESTING OF A CSF SAMPLE FROM A 20-MONTH-OLD MALE PATIENT. THE PUBLICATION SUGGESTED THAT THE BIOFIRE RESULT LED TO PREMATURE DISCONTINUATION OF ACYCLOVIR, WHICH CONTRIBUTED TO NEUROLOGICAL SEQUELAE, INCLUDING SPASTIC QUADRIPARESIS AND FUNCTIONAL DEPENDENCE. BIOFIRE HAS REQUESTED FURTHER INFORMATION FROM THE AUTHOR AND IS CURRENTLY INVESTIGATING THIS EVENT. NO REMEDIAL ACTION, CORRECTIVE ACTION, PREVENTIVE ACTION, OR FIELD SAFETY CORRECTIVE ACTION (FSCA) HAS BEEN DEEMED NECESSARY AT THIS TIME. UPDATE FOR SUPPLEMENTAL REPORT: UPON FOLLOWING WITH THE AUTHORS, THEY INDICATED THE STUDY SAMPLE WAS FROM OVER A YEAR AGO. THE FIRST FILMARRAY RESULT WAS FROM ANOTHER CLIENT (FUNDACIÓN CLUB NOEL), BUT THE CHILD HAD BEEN EXPERIENCING SYMPTOMS FOR LESS THAN 24 HOURS, SO THE POSSIBILITY OF A FALSE NEGATIVE WAS VERY HIGH. THE PATIENT DID NOT REQUIRE ANY FURTHER INTERVENTION AND SUFFERED NO LASTING EFFECTS FROM DISCONTINUING THE TREATMENT. BIOFIRE IS STILL INVESTIGATING THIS EVENT IN ORDER TO DETERMINE THE ROOT CAUSE. LEONG ET AL. PUBLISHED A STUDY "A LETHAL CASE OF CRYPTOCOCCAL MENINGITIS IN AN APPARENTLY IMMUNOCOMPETENT PATIENT: A CASE REPORT HIGHLIGHTING DIAGNOSTIC PITFALLS AND THERAPEUTIC CHALLENGES," WHICH DESCRIBES A POTENTIAL FALSE NEGATIVE CRYPTOCOCCUS NEOFORMANS/GATTII RESULT ON THE FILMARRAY MENINGITIS/ENCEPHALITIS (ME) PANEL DURING PATIENT TESTING. THE PUBLICATION SUGGESTED THAT THE BIOFIRE RESULT MAY HAVE DIRECTLY CONTRIBUTED TO A DELAY IN DIAGNOSIS. THE PUBLICATION FURTHER CLAIMED THAT THE DIAGNOSTIC DIFFICULTIES ENCOUNTERED WITH THIS PATIENT EVENTUALLY LEAD TO THE DELAYED DIAGNOSIS AND MORTALITY. THE AUTHORS WERE UNABLE TO PROVIDE ANY ADDITIONAL INFORMATION REGARDING PATIENT IMPACT. THE INVESTIGATION CONCLUDED THAT THE MOST LIKELY CAUSE OF THE POTENTIAL FALSE NEGATIVE RESULT WAS LOW-LEVEL ORGANISM/NUCLEIC ACID WITHIN THE SAMPLE WHICH WAS NEAR/BELOW THE LIMIT OF DETECTION (LOD) FOR THE BIOFIRE ME PANEL.
