We found 19 models of Momentum by Ulrich Medical Usa, Inc. in the FDA's device registry (GUDID). Together they are named in 78 adverse event reports in the FDA's MAUDE database, received between Jul 15, 2020 and Jun 12, 2026 — classified as 4 injuries, 74 malfunctions.
The most frequently reported specific device problem is “Material Fragmentation” (59 reports).
Reports come from manufacturers, hospitals and patients and are not verified by the FDA. A report does not prove the device caused the event. Widely used products collect more reports, so totals can't be used to compare products or estimate failure rates. Only reports that name a model's exact device identifier are counted. MAUDE report 25479832 ↗MalfunctionJun 11, 2026Mechanical Problem; Detachment of Device or Device Component
TULIP DISASSOCIATED FROM THE POLYAXIAL SCREW HEAD WHILE THE SURGEON WAS IMPLANTING IT. THE SCREW WAS REMOVED AND REPLACED WITHOUT INCIDENT AND THERE WAS A SHORT DELAY IN THE PROCEDURE.
Patient problems: Failure of Implant

DURING A CASE, THE SURGEON BROKE 5 DRIVERS IN THE ATTEMPT TO INSTALL AND THEN REMOVE A SCREW. THE SURGEON DID NOT TAP LINE TO LINE PRIOR TO IMPLANTING THE SCREW. AFTER BREAKING ALL DRIVERS, THE SURGEON USED ANOTHER MANUFACTURER'S INSTRUMENT TO REALIGN THE TRAJECTORY OF A SCREW AND BEGAN HAMMERING ON THE INSTRUMENT WHICH CAUSED THE SCREW TO BREAK. ALL PIECES OF THE SCREW WERE RETRIEVED BUT WERE NOT RETURNED TO UUSA. THE PATIENT WAS NOT AFFECTED. NO FURTHER INFORMATION WAS AVAILABLE FOR THE MANUFACTURER OF THE SCREW USED. THE AVAILABLE INFORMATION DID NOT ALLOW US TO DETERMINE WHETHER A SERIOUS INJURY COULD RESULT FROM THE DRIVER BREAKAGES, A MEDICAL DEVICE REPORT WAS SUBMITTED FOR EACH SCREWDRIVER.

MAUDE report 22310553 ↗InjuryApr 17, 2025Break; Break
THE PATIENT COMPLAINED OF PAIN; AFTER DOING A CAT SCAN THE SURGEON BECAME CONCERNED REGARDING THE "WEIRD PRESENTATION" OF THE SCREWS AND OPTED TO PERFORM A MINI-REVISION ON 3/20. DURING THE CASE ON 3/20 X3 TOTAL SCREWS WERE BROKEN, 2 5.5X45MM PEDICLE SCREWS (BROKEN AT THE TIP) AND A 9.0X90MM ILIAC SCREW (BROKEN NEAR THE TOP OF THE SCREW BUT BELOW THE TULIP HEAD). CEMENT WAS NOT USED WHEN THESE SCREWS WERE IMPLANTED AND THE TIPS OF BOTH PEDICLE SCREWS WERE LEFT IN THE PATIENT AS WAS THE ILIAC SCREW. THE PEDICLE SCREWS HAD BEEN IN THE PATIENT SINCE 2022 AND THE ILIAC SCREW HAD BEEN IN SINCE 2021. THERE IS NO EVIDENCE TO SUGGEST THAT THESE SCREWS WERE BROKEN PRIOR TO THEIR ATTEMPTED REMOVAL ON (B)(6) AND THE SURGEON ONLY PERFORMED THIS REVISION DUE TO THE PATIENT COMPLAINING OF PAIN AND THE PRESENTATION OF THE SCREWS ON THE CAT SCAN.
Patient problems: Back pain
