  IT WAS BY UNITED STATES REPORTED THAT DURING SERVICE AND EVALUATION, IT WAS DETERMINED THAT THE CUTTER DEVICE WAS FOUND TO BE DULL/WOULD NOT CUT. IT WAS NOTED IN THE SERVICE ORDER THAT THE SAW DEVICE (CUTTER) WAS STUCK IN THE ATTACHMENT DEVICE. THIS EVENT DID NOT OCCUR DURING SURGERY. THERE WAS NO PATIENT INVOLVEMENT. THERE WERE NO REPORTS OF INJURIES, MEDICAL INTERVENTION OR PROLONGED HOSPITALIZATION. THE EXACT DATE OF THE EVENT WAS UNKNOWN. HOWEVER, IT WAS REPORTED THAT THE EVENT OCCURRED IN 2022.   IT WAS REPORTED FROM (B)(6) THAT DURING AN UNSPECIFIED CRANIOTOMY SURGICAL PROCEDURE, IT WAS DISCOVERED THAT THE CUTTER DEVICE ¿WAS PROBABLY BROKEN¿. ACCORDING TO THE REPORTER, THE PROCEDURE WAS COMPLETED SUCCESSFULLY WITH A FIFTEEN-MINUTE DELAY. IT WAS NOT REPORTED IF A SPARE DEVICE WAS AVAILABLE FOR USE. IT WAS FURTHER REPORTED THAT THE POSTOPERATIVE MRI (MAGNETIC RESONANCE IMAGING) PERFORMED THE FOLLOWING DAY SHOWED A METAL ARTIFACT IN THE FIELD OF THE CRANIOTOMY. A REVISION SURGERY WAS THEN PERFORMED TO REMOVE THE ARTIFACT. IT WAS REPORTED THAT THE PATIENT RECOVERED. THERE WAS PATIENT INVOLVEMENT REPORTED. THERE WAS A MEDICAL INTERVENTION REQUIRED AS A RESULT OF THIS EVENT. THE EXACT DATE OF THE EVENT WAS UNKNOWN. HOWEVER, IT WAS REPORTED THAT THE EVENT OCCURRED IN 2019.
