  IT WAS REPORTED FROM SWITZERLAND THAT DURING SERVICE AND EVALUATION, IT WAS DETERMINED THAT THE CUTTER DEVICE FAILED VISUAL INSPECTION DUE TO A WORN CUTTING EDGE (CUTTER DAMAGED). IT WAS FURTHER DETERMINED THAT THE DEVICE FAILED PRETEST FOR CHECK FOR SAFETY, CHECK FOR UNTRUE RUNNING AND CHECK FOR GENERAL CONDITION. IT WAS NOTED IN THE SERVICE ORDER THAT THE CUTTER DEVICE COULD NOT DISENGAGE FROM THE ATTACHMENT AND MOTOR DEVICE. THIS EVENT DID NOT OCCUR DURING SURGERY. THERE WAS NO PATIENT INVOLVEMENT. THERE WERE NO REPORTS OF INJURIES, MEDICAL INTERVENTION OR PROLONGED HOSPITALIZATION. ALL AVAILABLE INFORMATION HAS BEEN DISCLOSED.   IT WAS REPORTED FROM (B)(6) THAT DURING SERVICE AND EVALUATION, IT WAS DETERMINED THAT THE CUTTER DEVICE COATING WAS PARTIALLY DAMAGED AND TRACES OF RESIDUE COULD BE FOUND ON THE SHAFT. IT WAS NOTED IN THE SERVICE ORDER THAT THE DEVICE HAD BEARING DAMAGE AND THE BURR COULD NOT BE DETACHED. IT WAS ALSO REPORTED THAT THE DEVICE WAS BEING USED WITH AN ATTACHMENT DEVICE. THIS EVENT DID NOT OCCUR DURING SURGERY. THERE WAS NO PATIENT INVOLVEMENT. THERE WERE NO REPORTS OF INJURIES, MEDICAL INTERVENTION OR PROLONGED HOSPITALIZATION.
