SIEMENS HEALTHINEERS BECAME AWARE OF A PATIENT INCIDENT INVOLVING THE MAGNETOM SYMPHONY A TIM SYSTEM. A PATIENT IN CRITICAL CONDITION WAS BROUGHT INTO THE MRI EXAMINATION ROOM WITH FOUR ATTACHED PERFUSOR PUMPS WITH STABILIZING MEDICATION. THE PERFUSORS WERE ATTRACTED BY THE MAGNET AND ENDED UP ON THE SIDE OF THE FRONT COVER. THE IMPACT DAMAGED THE PERFUSORS, SO THE NECESSARY MEDICATION WAS NOT ADMINISTERED. IT IS UNKNOWN, AS OF THE DATE OF THIS REPORT, IF THIS EVENT MAY HAVE CONTRIBUTED TO THE PATIENT'S DEATH SHORTLY AFTER THE INCIDENT.

SIEMENS HEALTHINEERS WAS INFORMED THAT ON TWO OCCASIONS, THE MR TECHNICIANS WERE CLOSE TO THE PATIENT EMERGENCY BALL INTERCOM WHEN THE PATIENTS SIGNALED THE TECHNICIANS. THE TECHNICIANS STATED THAT THESE SIGNAL TONE INCIDENTS HAVE CAUSED PROBLEMS WITH THEIR EARS. ONE INCIDENT OCCURRED WHEN THE INTERCOM WAS POSITIONED SO THAT IT WAS ELEVATED NEAR THE TECHNICIAN¿S EAR LEVEL. WHY THE INTERCOM WAS ELEVATED AT THIS HEIGHT IS UNKNOWN. AFTER A PATIENT PRESSED THE SQUEEZE BALL, THE SIGNAL TONE SOUNDED FROM THE INTERCOM. THE MR TECHNICIAN CLAIMS TO HAVE HAD RINGER IN THE AFFECTED EAR AS A RESULTS. IT IS UNKNOWN IF THE MR TECHNICIAN WAS MEDICALLY EXAMINED OR TREATED, OR FOR HOW LONG THE RINGING IN THEIR EAR CONTINUED. HOWEVER, IT WAS CLARIFIED THAT THE RINGING WAS TEMPORARY. THE SECOND INCIDENT OCCURRED WHEN A DIFFERENT MR TECHNICIAN¿S HEAD WAS VERY CLOSE TO THE INTERCOM. WHEN COMMUNICATING WITH THE PATIENT VIA THE INTERCOM, THIS MR TECHNICIAN HAS THE HABIT OF LEANING TOWARDS THE INTERCOM SO THEY CAN SPEAK DIRECTLY INTO THE INTERCOM, WITH LITTLE DISTANCE BETWEEN THE INTERCOM AND THEIR HEAD. A PATIENT PRESSED THE SQUEEZE BALL WHEN THE MR TECHNICIAN WAS EXACTLY IN THIS POSITION. AS A RESULTS,

IT WAS REPORTED THAT WHILE USING THE HOLOGIC BREAST COIL DURING A PATIENT EXAMINATION THE TABLE DID NOT STOP WHILE MOVING INTO THE BORE. THIS ISSUE RESULTED IN COMPRESSION OF THE PATIENT AND BROKEN RIBS. A TOTAL OF SIX (6) RIBS WERE BROKEN, THREE (3) ON EACH SIDE. THE SYSTEM SHOWED NO ERRORS AT THE TIME OF THE EVENT. AN OBSTRUCTION TEST ON LONGITUDINAL TABLE MOVEMENT INTO BORE WAS PERFORMED WITH THE FOLLOWING RESULTS: THE TABLE DRIVE POWER WAS INTERRUPTED, AND THE TABLE STOPPED WHEN OBSTRUCTED.

    Patient problems: Bone Fracture(s)

SIEMENS BECAME AWARE OF AN INCIDENT THAT OCCURRED ON THE MAGNETOM SYMPHONY UNIT. SIEMENS CUSTOMER SERVICE ENGINEER (CSE) MOUNTED THE CURRENT LEAD INTO THE MAGNET AND CHECKED THE MAG VOLTAGE TO RAMP DOWN THE SYSTEM. THE MAG VOLTAGE WAS OUT OF TOLERANCE, THEREFORE, THE ENGINEER TRIED TO TAKE OUT THE CURRENT LEAD; DURING THIS ACTIVITY A SUDDEN SYSTEM QUENCH STARTED. WHEN QUENCH OCCURRED, THE ENGINEER TRIED TO CLOSE THE CURRENT LEAD PORT BUT LOST CONSCIOUSNESS (POSSIBLY DUE TO LOW OXYGEN LEVELS CAUSED BY HIGH HELIUM LEAK). THE ENGINEER FELL DOWN THE MAGNET LADDER TO THE FLOOR AND SUFFERED A COLD BURN TO THE RIGHT HAND AS WELL AS NUMBNESS AND LOSS OF SENSATION IN HIS LEFT LEG. THE COLD BURN WAS TREATED WITH ANTIBIOTIC BURN CREAM. A NEUROLOGIST AND ORTHOPEDIST EXAMINED THE LEFT LEG AND PERFORMED AN MRI SCAN OF THE LUMBER. THE ENGINEER HAD TO RECEIVE PHYSICAL THERAPY FOLLOWING THE INCIDENT. HOWEVER, THE TREATMENT DID NOT HAVE THE EXPECTED EFFECT. THE ENGINEER CONTINUES TO EXPERIENCE NUMBNESS AND LOSS OF SENSATION IN THE LEFT LEG. THEREFORE, IT WAS DECIDED TO PERFORM A SURGERY AFTER THE COVID PANDEMIC. THE INCIDENT HAS ALREADY BEEN INVESTIGATED. THE INCIDENT OCCURRED DUE TO A WORKING ERROR

Patient problems: Burn(s); Cryogenic Burn; Numbness
