IT WAS REPORTED THAT THE TIP OF AN ASAHI GUIDE WIRE SEPARATED DURING A PCI TO TREAT A EXTREMELY TORTUOUS, SEVERELY CALCIFIED CTO IN THE PROXIMAL RCA. ACCORDING TO THE PHYSICIAN, THE EXISTING STENT HAD BEEN PLACED YEARS EARLIER AND WAS PROBABLY FRACTURED. ATTEMPTS WERE MADE WITH SEVERAL WIRES WITH A 2.0 BALLOON FOR SUPPORT; HOWEVER, ALL MET WITH NO SUCCESS. WHILE USING THE ASAHI GUIDE WIRE, IT WAS PROLAPSED BUT WAS SUBLUMINAL. WITH THE TORTUOSITY, THE HEAVY CALCIFICATION AND THE EXISTING STENT, THE RADIOPAQUE PORTION OF THE WIRE BROKE OFF AND WAS LEFT IN THE SUBINTIMAL SPACE. THE PIECE OF WIRE THAT WAS LEFT IN THE SUBINTIMAL SPACE WAS NOT MOVING AND ACCORDING TO THE PHYSICIAN, POSED NO THREAT TO THE PATIENT. A NEW NON-ASAHI WIRE WAS USED SUBSEQUENTLY WITH NO SUCCESS IN CROSSING AND THE CASE FINISHED.
