InjurySep 10, 2026Material Separation
It was reported that an Asahi Gladius Mongo 14 ES guide wire was used during a plain old balloon angioplasty (POBA) to treat a heavily calcified, heavily tortuous chronic total occlusion (CTO) in the mid anterior tibial artery (ATA). The ATA was totally occluded throughout its entire length from the proximal segment. The Gladius Mongo 14 ES guide wire was advanced into the proximal ATA. While advancing the tip of the guide wire to make a small J shape, it got caught and entangled, forming a knot in one of the side branches of the ATA branching off the proximal segment. Subsequently, the entangled portion of the guide wire could not be unknotted and withdrawn. The tip of the Gladius Mongo 14 ES guide wire broke off within the small side branch of the ATA. After the tip fracture, the proximal segment of the Gladius Mongo 14 ES guide wire was successfully withdrawn from the patient anatomy. The separated wire tip remained in the patient anatomy and could not be retrieved. The physician commented that the tip of the guide wire became entangled and formed a knot as a result ofrapid advancement and rotation of the wire within an extremely narrow side branch of the ATA. IT WAS REPORTED THAT AN ASAHI GLADIUS MG14 ES GUIDE WIRE WAS USED FOR REVASCULARIZATION OF A MILDLY CALCIFIED 100% OCCLUDED LESION IN THE ULNAR ARTERY. THE GUIDE WIRE WAS EXCESSIVELY TORQUED AND KNOTTED UP IN THE DISTAL ULNAR ARTERY. DURING REMOVAL ATTEMPTS, A DISTAL COIL SEGMENT WAS DETACHED FROM THE PROXIMAL SIDE AND LEFT IN THE PATIENT ANATOMY. THE PHYSICIAN CONCLUDED THAT THE WIRE FRAGMENT COULD BE LEFT IN SITU WITHOUT ISSUE. IT WAS INFORMED THAT THE PATIENT WAS DISCHARGED, WHOSE CONDITION WAS THE SAME AS THAT BEFORE THE PROCEDURE ALTHOUGH THE INTENDED REVASCULARIZATION WAS NOT ACHIEVED.

IT WAS REPORTED THAT A PERIPHERAL PERCUTANEOUS INTERVENTION (PPI) WAS PERFORMED FOR A HEAVILY CALCIFIED, MODERATELY TORTUOUS, AND 90-99% OCCLUDED LESION IN THE ANTERIOR TIBIAL ARTERY (ATA). WHEN AN ATTEMPT WAS MADE WITH AN ASAHI GLADIUS MONGO 14 ES GUIDE WIRE, INSERTED IN AN ASAHI CROSSLEAD PERIPHERAL SUPPORT CATHETER, TO PERFORM PEDAL LOOP REVASCULARIZATION. DURING PROCEDURE, THE SUBJECT GLADIUS MONGO 14 ES GUIDE WIRE WAS CAUGHT IN THE DISTAL TIBIAL ARTERY (TA). AFTER SEVERAL WITHDRAWAL ATTEMPTS WITH DIFFICULTY, THE GUIDE WIRE WAS REMOVED BUT FOUND ITS DISTAL SEGMENT DETACHED. AN UNSPECIFIED MICRO SNARE WAS USED AND THE WIRE FRAGMENT WAS SUCCESSFULLY RETRIEVED. IT WAS INFORMED THAT THERE WERE NO ADVERSE PATIENT EFFECTS AND THE PROCEDURE WAS TERMINATED. THE PATIENT WAS REPORTEDLY DISCHARGED AFTER HEMOSTASIS WAS ACHIEVED.

WHILE WORKING ON VERY CALCIFIC AND DIFFUSELY DISEASED ANTERIOR TIBIAL ARTERY, THE MONGO ES .014 WIRE BECAME TRAPPED IN THE DISTAL AT BY THE PEDAL LOOP WHILE IN THE .014 170CM CROSSWALK. IT WAS NOTED THAT WHEN THE WIRE WAS FINALLY ABLE TO BE REMOVED, THE DISTAL TIP OF THE WIRE WAS DISCONNECTED FROM THE REST OF THE WIRE. A MICRO SNARE WAS USED TO SUCCESSFULLY REMOVE THE BROKEN PIECE OF THE WIRE. NO ISSUES WERE NOTED, AND CASE WAS TERMINATED. PATIENT WAS RECOVERED IN THE RECOVERY AREA UNTIL HEMOSTASIS OF FEMORAL SITE WAS SEALED, THEN DISCHARGED.

MD KNUCKLED WIRE WITH IN RCA CTO AND UPON REMOVING JACKETED PORTION CAME OFF CORE WIRE. THE JACKETED PORTION BROKE OFF IN THE SUBINTIMAL SPACE AND WAS UNRETRIEVABLE. THERE WAS NO SIDE EFFECTS. THEY WERE UNABLE TO CROSS DUE TO TOUGH CTO AND THE AMOUNT OF RADIATION, SO THE SECOND PROCEDURE WAS PERFORMED LATER. IN THE SECOND PROCEDURE, THE CTO WAS CROSSED FROM RETROGRADE APPROACH AND STENTED.

DR (B)(6) WAS TRYING TO NAVIGATE AN EXTREMELY CALCIFIED AT CTO WITH NO BLOOD FLOW. HE TRIED TRAVERSING AROUND THE SEGMENT AND WIRE BECAME STUCK AND WHEN HE TRIED TO ADVANCE ABOUT A 1CM PORTION OF THE DISTAL TIP CAME OFF. BECAUSE OF WHERE THE TIP BROKE AND NO FLOW IT WAS DETERMINED BY PHYSICIAN THAT LEAVING WHERE IT WAS WOULD NOT POSE ANY SORT OF RISK TO PATIENT.

IT WAS REPORTED THAT A PERIPHERAL PERCUTANEOUS INTERVENTION (PPI) WAS PERFORMED FOR A HEAVILY CALCIFIED AND MODERATELY TORTUOUS CHRONIC TOTAL OCCLUSION (CTO) IN THE TIBIAL ARTERY (TA). WHEN AN ATTEMPT WAS MADE WITH AN ASAHI GLADIUS MONGO 14 ES GUIDE WIRE TO CROSS THE LESION, THE GUIDE WIRE WAS CAUGHT BY THE LESION. AS FURTHER ATTEMPTS WERE MADE TO ADVANCE THE GUIDE WIRE, IT BROKE OFF FOR APPROXIMATELY 1CM FROM THE WIRE TIP. HAVING CONSIDERED THAT THERE HAD BEEN NO FLOW IN THE SEGMENT WHERE THE WIRE WAS DETACHED, THE PHYSICIAN CONCLUDED THAT THE FRAGMENT COULD BE LEFT IN SITU AS THERE WOULD BE NO RISK TO THE PATIENT. THE PROCEDURE WAS COMPLETED WITHOUT PROBLEM, AND THE PATIENT WAS DISCHARGED IN A FEW HOURS AFTER THE PROCEDURE.

SFA, POPLITEAL, RT TIBIOPERONEAL TRUNK EXTENDING THROUGHOUT RT POST TIBIAL ARTERY. CONTRALATERAL PLACEMENT OF SHEATH TO RT SFA COMPLETED AND MONGO14 ES USED AS INITIAL PERIPHERAL GUIDE WIRE. AFTER SUCCESSFULLY NAVIGATING THROUGH SFA, POPLITEAL AND TIBIOPERONEAL TRUNK AND MOST OF THE RT POST TIB. WIRE ADVANCEMENT WAS STOPPED, CROSSWALK ADVANCED AND MONGO WIRE WAS REMOVED WITH THE INTENT TO TRY ANOTHER WIRE TO ADVANCE. UPON REVIEW OF WIRE POST REMOVAL, THE TIP OF THE WIRE EXCLUDING THE CORE WAS STRIPPED FROM THE WIRE. UPON ANGIOGRAPHIC REVIEW, THE TIP OF WIRE WAS LEFT IN THE DISTAL POSTERIOR TIBIAL ARTERY AT THE BEGINNING OF THE CTO. THE PHYSICIAN AND I DISCUSSED THE FINDING AND HE STATED THAT IT WOULD CAUSE NO FURTHER HARM TO THE PATIENT BECAUSE OF THE EXISTING CTO.
