MalfunctionSep 17, 2026Retraction Problem
It was reported that the patient presented to the hospital for a scheduled implant procedure. During the procedure, the lead helix was unable to retract despite several attempts and using mulitple helix locking tools. The lead was removed and replaced. The patient remained stable.

MAUDE report 50791206 ↗InjurySep 14, 2026Failure to Capture; Low impedance
It was reported that the patient presented to the Emergency Room (ER) experiencing syncope and asystole. Device interrogation revealed that the right ventricular (RV) lead exhibited loss of capture in both bipolar and unipolar configurations and decreased pacing impedance trend. Temporary pacemaker was placed; the RV lead was subsequently explanted and replaced with a high voltage lead for implantable cardioverter defibrillator upgrade due to low ejection fraction. No lead damage was visualized. The patient was in stable condition.
