Summary
The FDA's MAUDE database contains 2 adverse event reports that list this exact device (DI 05706069870510), received between Mar 22, 2024 and Sep 5, 2025 — classified as 2 malfunctions.
The most frequently reported specific device problem is “Material Deformation” (1 report).
Reports come from manufacturers, hospitals and patients and are not verified by the FDA. A report does not prove the device caused the event, and counts can't be used to compare devices or estimate failure rates. What reports can and can't tell you · How this data works · Data updated Oct 18, 2026.

RESULTS OF TECHNICAL INVESTIGATION: THE BATTERY WAS NOT RETURNED, SO ITS ROLE IN THE INCIDENT CANNOT BE CONFIRMED. EVIDENCE SUGGESTS THE THERMAL EVENT ORIGINATED NEAR THE BOTTOM OF THE DEVICE, SPECIFICALLY IN THE POWER SUPPLY MODULE (PSM) AREA. IT IS IMPORTANT TO NOTE THAT THE PSM WAS NOT RETURNED FOR FURTHER EVALUATION. THE RISK OF A BATTERY CELL SHORTING LEADING TO THE MELTING OF THE HEARING AID HOUSING APPEARS TO BE LOW. A CONCLUSIVE DETERMINATION REGARDING A POTENTIAL DEVICE MALFUNCTION CANNOT BE MADE WITHOUT THE RETURN OF ALL RELEVANT ITEMS AND COMPONENTS. THERE ARE POSSIBLY TWO EXPLANATIONS IN THIS CASE; THE OVERHEATING WAS CAUSED BY EITHER AN EXTERNAL HEAT SOURCE OR FROM A THERMAL RUNAWAY EVENT RESULTING IN THE RAPID RELEASE OF HEAT THAT CHARRED AND DEFORMED NEARBY PLASTIC COMPONENTS. THE PRESENCE OF BURN MARKS ON THE CHARGER SUPPORTS EXTERNAL HEATING. WE WERE INFORMED BY THE USER THAT THE DEVICE WAS PLACED ON A MICROWAVE. THE CHARGER IS STILL FUNCTIONAL AFTER THIS EVENT. ADDITIONALLY, NO FURTHER INFORMATION REGARDING THE BURNED FINGER HAS BEEN RECEIVED FROM THE USER, AND THERE HAVE BEEN NO REPORTS OF MEDICAL INTERVENTION.

IN THIS EVENT, THE PATIENT'S HEARING AIDS AND CHARGER MELTED WHILE CHARGING. INVESTIGATION RESULTS: CHARGER WITH ACCESSORIES AND HEARING AIDS WERE RETURNED FOR INVESTIGATION. NO SIGN OF MELTED OR BURNT PART ON THE EXTERNAL AREA OF THE CHARGER. POWER ADAPTER AND CABLE CONNECTOR HAVE NO VISUAL DEFECT AND NO SIGN OF BURNT/ MELTED PART. POWER ADAPTER HAS GOOD VOLTAGE OUTPUT BUT IS NOT A WSA PRODUCT. SAFETY INFORMATION IN USER GUIDE, INSTRUCTS USER TO ONLY USE A WIDEX POWER ADAPTER AND CABLE WITH CHARGER. CANNOT DIAGNOSE HEARING AIDS AS BOTH ARE ALREADY STUCK INSIDE THE CHARGING WELLS, WITH SOME PARTS MELTED. ONE OF THE HEARING AID'S BATTERY ALREADY EXPLODED. INTERNAL COMPONENTS OF THE CHARGER HAS SIGNS OF MELTING/BURNING. 2ND LEVEL INVESTIGATION TO BE PERFORMED TO DETERMINE WHAT WENT WRONG IN THE CHARGER'S INTERNAL COMPONENTS.
Patient problems: No Clinical Signs, Symptoms or Conditions
