Quick summary

This is a BD SmartSite blood infusion set, tubing used to give blood transfusions. The 35 reports mostly describe leaking — blood or fluid escaping from the tubing, drip chamber or pumping segment, sometimes because the plastic cracked or the parts separated. Reports also mention air-in-line alarms and faster-than-expected flow. There are no deaths and one injury. One recall, started May 2019 and closed, involved wall thickness in the pumping segment that could allow medication or blood to be delivered when the pump isn't running.

An incomplete occlusion can be caused by a variation in the wall thickness of the pumping segment of the affected infusion sets used with the. The issue has the potential to lead to unintended delivery of medication when the pump module is not in running status or can result in faster than expected drug delivery flow when the pump is infusing.
