It was reported to Boston Scientific Corporation (BSC) that a CRE PRO Wireguided DASE Dilatation Balloon was attempted to be used in the esophagus during an esophageal stricture procedure performed on (b)(6) 2026.During the procedure, resistance was encountered when threading the balloon through the working channel of the scope. Upon inflation with the syringe assembly, the balloon burst inside the patients esophagus. The customer stated that no pieces of the balloon detached inside the patient. The procedure was completed with another CRE PRO Wireguided DASE Dilatation Balloon of a different lot number and a non-BSC inflation device. There were no patient complications reported as a result of this event. The patients condition at the conclusion of the procedure was reported to be stable.Additional Information received 03 SEP 2026.Patient was having an Esophagogastroduodenoscopy (EGD) Dilation and a CRE Pro Wireguided dilator measurements 18-20mm was used. Instrument appeared intact, all checks completed. The balloon was inflated as normal, but the balloon burst while in the patient's esophagus at the max pressure. No injury to the patient, however, a new instrument had to be used d

NOTE: THIS REPORT PERTAINS TO ONE OF TWO DEVICES THAT WERE USED IN THE SAME PATIENT AND PROCEDURE. IT WAS REPORTED TO BOSTON SCIENTIFIC CORPORATION THAT AN CRE PRO WIREGUIDED DASE DILATATION BALLOON WAS ATTEMPTED TO BE USED IN THE ESOPHAGUS DURING A GASTROSCOPY WITH DILATATION PROCEDURE FOR ESOPHAGEAL STRICTURE PERFORMED ON (B)(6) 2026. DURING PROCEDURE, AFTER THE BALLOON WAS INTRODUCED THROUGH THE WORKING CHANNEL, THE FIRST TWO DILATION STAGES (18MM AND 19MM) WERE PERFORMED WITHOUT ANY DIFFICULTIES. DURING THE THIRD DILATION STAGE (20 MM), THE BALLOON WAS NO LONGER ABLE TO MAINTAIN THE FLUID DUE TO A POSSIBLE LEAK IN THE DEVICE. THE BALLOON WAS REMOVED AND A SECOND OF THE SAME BALLOON DEVICE WAS ATTEMPTED TO BE USED. HOWEVER, THE SAME PROBLEM OCCURRED WITH THE SECOND BALLOON. THE PROCEDURE WAS COMPLETED SUCCESSFULLY WITH A THIRD CRE PRO WIREGUIDED DASE DILATATION BALLOON. THE PATIENT REMAINED STABLE THROUGHOUT THE ENTIRE PROCEDURE AND THERE WERE NO PATIENT COMPLICATIONS REPORTED AS A RESULT OF THIS EVENT. THE PATIENT'S CONDITION AT THE CONCLUSION OF THE PROCEDURE WAS REPORTED TO BE FULLY RECOVERED.

IT WAS REPORTED TO BOSTON SCIENTIFIC CORPORATION THAT A CRE PRO WIREGUIDED DILATATION BALLOON WAS USED IN THE ESOPHAGUS DURING AN ESOPHAGOGASTRODUODENOSCOPY (EGD) PROCEDURE TO TREAT STRICTURE PERFORMED ON (B)(6) 2026. DURING THE PROCEDURE, THE PHYSICIAN INSTRUCTED INFLATION OF THE BALLOON TO 18 MM, WHICH WAS PERFORMED WITHOUT RESISTANCE. THE PHYSICIAN THEN DIRECTED FURTHER INFLATION TO 20 MM. WHILE SLOWLY INFLATING PAST 19 MM AND APPROACHING 20 MM, THE BALLOON RUPTURED. THE PROBLEM WAS SIGNIFICANT THAT THE BALLOON MATERIAL SPLIT, RESULTING IN AN IMMEDIATE LOSS OF FLUID AND PRESSURE. THE CUSTOMER STATED THAT NO PIECES OF THE BALLOON DETACHED INSIDE THE PATIENT. THE PROCEDURE WAS COMPLETED WITH ANOTHER OF THE SAME DEVICE. THERE WERE NO PATIENT COMPLICATIONS REPORTED AS A RESULT OF THIS EVENT.
