DURING SURGERY, THE SURGEON NEEDED TO USE THE HUMERAL BLAZER A COUPLE TIMES DUE TO VERY DENSE BONE. IT WAS WORKING FINE AT THE BEGINNING, AND TOWARD THE END THE CANNULATED HANDLE WOULD NO LONGER THREAD INTO THE BLAZER TO ATTACH. UPON LOOKING AT THE THREADS, THE HANDLE WAS FINE BUT THE THREADS WITHIN THE BLAZER WERE VISIBLY DAMAGED AND COULD NO LONGER BE USED. SURGERY WAS DELAYED 5 MINUTES DUE TO THE REPORTED EVENT.
