DEVICE REPORT FROM SYNTHES REPORTS AN EVENT IN JAPAN AS FOLLOWS: IT WAS REPORTED THAT THIS WAS A PCF PERFORMED ON (B)(6) 2022. A SCREW ORIGINALLY CAME WITH AN OFFSET CONNECTOR. TWO CONNECTORS WERE UNSEALED. IT WAS FOUND THAT THE SCREW ON ONE OF THE CONNECTORS HAD BEEN ATTACHED UPSIDE DOWN TO THE CONNECTOR. THEY DETACHED THE SCREW WITH A PLIER. ANOTHER CONNECTER HAD NO ISSUE. THE PROCEDURE WAS COMPLETED WITHOUT SURGICAL DELAY. NO FURTHER INFORMATION IS AVAILABLE. THIS REPORT IS FOR ONE (1) LATERAL OFFSET CON 4.0 S. THIS IS REPORT 1 OF 1 FOR COMPLAINT (B)(4).
